This addendum forms a part of the specifications and modifies or clarifies the original
specifications as noted below. The offeror shall acknowledge receipt of this addendum on
its Certification of RFSQ. Failure to do so may result in the offeror to disqualification. This
addendum consist of 3 pages.

Edit, Page 32 EXHIBIT H: 4.4.5 CRITERION COMPARABLE PROJECTS
The purpose of this section is to obtain Information about Comparable Projects completed
by the DBE and DBE Members. The responses provided will be used to evaluate Criterion
4.4.5 Comparable Projects (20 Points Possible).
To clarify the intent of this provision for the Criterion for Comparable Projects for
Points Possible on the RFSQ. This explanation is provided for clarification and does
not modify the terms of the Agreement. Points Possible stated on page 32 should
read as follows: Criterion 4.4.5 Comparable Projects (30 Points Possible).

The purpose of this section is to obtain information about the DBE and DBE Members. The
responses provided will be used to evaluate Criterion 4.4.3 DBE History (15 Points
Possible).
To clarify the intent of this provision for the Criterion for DBE History for Points
Possible on the RFSQ. This explanation is provided for clarification and does not
modify the terms of the Agreement. Points Possible stated on page 26 should read
as follows: Criterion 4.4.3 DBE History (20 Points Possible).

Edit, Page 28 EXHIBIT G: CRITERION 4.4.4 QUESTIONNAIRE
The purpose of this section is to obtain information about the qualifications and experience
of the DBE. The responses provided will be used to evaluate Criterion 4.4.4
Questionnaire (45 Points Possible). Provide a response to the questions below for the
DBE and all DBE Members and/or DBE Participants, as applicable. For each response
noted as “Unfavorable” the DBE must attach a brief statement that identifies the party or
parties involved and fully explains the circumstance. Failure to complete this questionnaire
or to provide the additional statements as applicable may result in the SOQ being deemed
non-responsive and ineligible for consideration. Attach additional sheets as necessary.
To clarify the intent of this provision for the Criterion for Questionnaire for Points
Possible on the RFSQ. This explanation is provided for clarification and does not
modify the terms of the Agreement. Points Possible stated on page 28 should read
as follows: Criterion 4.4.4 QUESTIONNAIRE (50 Points Possible).
Summary of Questions and Clarifications:
Q1. RFSQ Section 2.4.2.3 requires certain Engineers to be listed, including
Mechanical Engineers and Electrical Engineers. May the Design-Build Entity list a
properly licensed Contractor in lieu of an Engineer for the design phase and also
the construction phase? For example, instead of an Electrical Engineer, may we
list a CSLB licensed C-10 Electrical Contractor?
A1: The RFSQ specifically requires that a qualified licensed Mechanical and Electrical
Engineer be listed to ensure compliance with the professional design requirements and
responsibilities. A C-10 contractor can perform or oversee the construction and installation
during construction phase.
Q2. RFSQ Section 1.6 states, “It is anticipated that, except as restricted by
applicable law, the District will reserve the right to require the Design-Builder to
conduct a competitive bidding process for all elements of the construction work.”
May the Design-Build entity list properly licensed contractors in its Design-Build
Team for design and construction? For example, may we identify in our proposal
a CSLB Licensed C-10 Electrical Contractor for Electrical design and
construction?
A2. The DBE can list properly licensed subcontractors or consultants for design and
construction services. The DBE will be responsible for ensuring that all subcontractors and
consultants are properly licensed for the project and the scope of work (e.g., a CSLB
Licensed C-10 Electrical Contractor for electrical design and construction).

Q3. The RFSQ specifies that Exhibit G (Questionnaire) must be completed for “the
DBE and all DBE Members and/or DBE Participants, as applicable”.
a) When the Design-Build Entity (DBE) itself is the General Contractor, is Exhibit
G required only for the General Contractor (as the DBE), or must it also be
completed by the Architect?
b) Are DBE Participants such as the surveyor, structural engineer, geotechnical
engineer, civil, mechanical, and electrical engineers also required to complete
Exhibit G individually?
A3. a) If the Architect is identified as a member of the DBE General Contractor, they do not
have to complete Exhibit G (Questionnaire). Only the DBE General Contractor is required
to complete Exhibit G (Questionnaire).
b) If all participants are identified in the proposal Only the DBE General Contractor is
required to complete Exhibit G (Questionnaire).
