The City of Perry Public Works is soliciting quotes from qualified contractors to supply quotes for the

installation of sod and landscape material for a landscape buffer along the city easement adjacent to

neighboring parcels as shown in the map, attached. The property is located at 625 and 623B Woodland

Drive Perry, GA 31069. Quotes should include all labor and material cost for the following

requirements:

1. As outlined in provided diagram, install sod and seed where designated after proper site

preparation and soil leveling. Finished site should be easily mow able and have total coverage of

soils with grass variety specified.

2. Install ornamental trees along edge of easement boundary as shown in the diagram (Green line

with arrows), in a triangle spacing with groups of 3 each, alternating species.

a. Crape Myrtle (hardy variety), minimum 8’ height

b. Sweet Bay Magnolia, minimum 8’ height

3. Procure locally grown and sourced plant material from vendors in the central Georgia area as

outlined in the planting plan.

Watering is not the responsibility of the contractor and will be handled by private property owners.

It is the City’s intention to award the entirety of the Scope of Services to one contractor. The selected

contractor must provide 1) a signed E-Verify affidavit and 2) proof of insurance in accordance with the

attached “Insurance Requirements” before a contract will be awarded.

“Insurance Requirements”

• CLIENT will require the CONTRACTOR to provide evidence of appropriate professional liability

insurance and workers’ compensation insurance coverage prior to commencement of work

• Such coverage must be provided by an insurance company or companies authorized to do business in

the State of Georgia with at least an “A” rating by A.M. Best.

• Certificates of Insurance must name the CLIENT as an Additional Insured, shall provide that the

CONTRACTOR’S policy is primary and not non-contributory over any insurance carried by the CLIENT,

waives subrogation and shall provide that the policy will not be cancelled or materially changed

without 30 days prior notice in writing to the CLIENT.

• Minimum insurance coverages required of the CONTRACTOR are as follows: General Liability of

$1,000,000.00, Vehicle Liability of $1,000,000.00 Workers’ Compensation at statutorily required limits

and Employers’ Liability of $1,000,000.00.
