An amendment has been issued that incoporates quantities in the solicitation.

This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-95 .

The Regional Health Contracting Office ? Atlantic , Army Health Contracting Activity , US Army Medical Command intends to solicit, negotiate, and award a Single-Award Blanket Purchase Agreement (BPA), however, the Government reserves the right to issue multiple awards for Compound Medications to be delivered to the Pharmacy at Keller Army Community Hospital (KACH), West Point, NY. The Contractor is to provide Compound Medications and adhere to all sterile preparation techniques and best practices. Compounded medications are to be sent to KACH in an expedited and cost effective manner. There are some products that must be procured as soon as possible due to the potential to impact life, limb or eyesight. These products may be picked up from the compounding pharmacy by KACH staff during normal business hours; therefore the physical location of the compounding pharmacy must be within 60 radius miles of KACH. See 52.212-1 quote marked Instruction to Offerors and 52.212-2 Evaluation of commercial items. All proposals received after the date of publication of this synopsis will be considered by the Government.

The ordering period will be from 1 October 2017 through 30 September 2022.

Clauses and provisions throughout this solicitation can be viewed by accessing website . FAR provision 52.212-1 [Instructions to Offerors Commercial Items] applies to this acquisition. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered in the System for Award Management (SAM) program. The SAM can be accessed via the internet at . Confirmation of SAM registration will be validated prior to awarding a contract. The provision at FAR 52.212-2, Evaluation ? Commercial Items and any addendums to this provision apply to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions to Implement Statutes or Executive Orders ? Commercial Items applies to this acquisition.

Vendors shall include a completed copy of FAR provision 52.212-3 Alt 1, Offeror Representations and Certifications with their offer. Offerors shall include a confirmation of receipt of any amendments to this solicitation. Failure to submit a confirmation could result in the offer being determined non- responsive. Clause 52.212-4 Contract Terms and Conditions-Commercial items, applies to this acquisition to include any applicable addenda.
