Combined Synopsis / Solicitation Posting: W81K0223Q0053





This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; request for quotes that are being requested and a written solicitation will not be issued.





This solicitation W81K0223Q0053 of Purchase Request BPAMAMC2023-2 is issued as a Request for Quote (RFQ).



Medical Readiness Contracting Office-Pacific, Joint Base Lewis-McChord (JBLM), Health Contracting Branch Lacey WA; intends to award multiple Blanket Purchase Agreements (BPAs) for the following source products:



Source: FDA – Approved; Human Tissue, Human Medical Cadavers to be provided to Madigan Army Medical Center, Anderson Simulation Center for GME Medical Surgical Resident certification & credentialling accreditation.



This BPA will be prepared in accordance with FAR Part 12 Commercial Items and FAR Part 13 Simplified Acquisition Procedures as supplemented with additional information included in this notice. The BPA when awarded will be issued for a maximum period of (5) years and can be cancelled by either party by written request giving (30) days’ notice.



Joint Base Lewis-McChord, Health Contracting Branch (JBLM-HCB) is requesting product and pricing as part of establishment of a Blanket Purchase Agreement (BPA) for the above listed FDA Human Tissue, Human Medical Cadavers, whole or in part, to be used for Surgical Residents on-hands Training & Surgical Skills Assessment clinical certification at Anderson Simulation Center, Madigan Army Medical Center Tacoma WA, indicated in this synopsis; from interested, FDA approved Vendors, who have the proper state and federal certifications both Large and Small Businesses.

Salient Characteristics

1. The Contractor must be able to provide fresh frozen whole human cadavers male / female and preserved to the Military Treatment Facility (MTF), to facilitate the instruction of Advanced Surgical Skills Assessment training in Trauma (ASSET), and Basic Endovascular Skills for Trauma (BEST) requirements & (CAPSTONE) for surgical and operational readiness, courses will be conducted between 4-5 times per Fiscal Year ( 01 October through 30 September) with a Human Tissue, Cadaver requirement of 3-4 for each training course.





2. The Contractor prior to shipment shall complete HIV, HEP C & Covid 19 testing and provide the results to the Anderson Simulation Center Course Director prior to shipment.





3. The Contractor shall provide proper chilled containers and coordinate delivery to the site at least 24 hours prior to the training course, and for return shipping, contractor to assume responsibility and final disposition of the Human Tissue, Human Medical Cadavers once returned to the Contractor location.





4. Madigan will provide to the Contractor a completed Tissue Request Form(contractor’s form) completed in advance indicating the Human Tissue, Human Cadaver requirements (full body, head & shoulder attached, male/female, age, tissue part, etc.)





5. The Contractor must have proper legal ethical authority, Certification and FDA Approval in handling as part of their primary organization mission through family donation / processing/shipment of Human Tissue,

Human Medical Cadavers under the Human Tissue Act of 2004 (HTA) and State of Washington Department of Health Services regulations.

The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2023-01. Simplified Acquisition Procedures will be followed. Interested parties must be registered in the System for Award Management (SAM), online at www.sam.gov.



All responsible Contractors who are interested shall fill out the attached Blanket Purchase Agreement Scope of Work and provide a separate price a product listing and submit by email.
