This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources (small and other than small) to perform a potential requirement. The Department of Veterans Affairs, Network Contracting Office 9, hereby provides notice of its anticipated issuance of an urgent Healthcare Service Contract to a qualified source. Based on responses received from this notice a determination will be made on the method used to solicit for proposals, proposal abstracts, or quotations. The anticipated period of performance shall consist of one (1) year beginning upon anticipated award date of 4/1/2023 through 3/31/2024. (See CLIN structure for anticipated need)
This urgent requirement will provide Onsite & On-call Board Certified/Board Eligible Hematology/Oncology Physician(s) Services for beneficiaries of the Robley Rex Veterans Affairs Medical Center (VAMC) located at 800 Zorn Ave., Louisville, KY, 40206-1433.
THIS SOURCES SOUGHT NOTICE IS NOT A REQUEST FOR COMPETITIVE QUOTES/PROPOSAL. However, interested parties may identify their interest and capabilities by responding to this requirement no later than January 25, 2023 at 10:00 AM, Central Time (CT).
Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. A determination not to compete this requirement based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for determining whether to conduct a competitive procurement.
The Department of Veterans Affairs will NOT be responsible for any costs incurred by interested parties in responding to this notice. Only written responses will be considered. All responses from responsible sources will be fully considered. As a result of analyzing responses to this notice, the Government shall determine if a solicitation will be issued. Any prospective contractor must be registered in the System for Award Management (SAM) to be eligible for award.

SCHEDULE OF SERVICES
The Contractor shall furnish all personnel to provide services necessary to perform Onsite & On-call Hematology/Oncology Physician Services to eligible beneficiaries of the Robley Rex Veterans Affairs Medical Center (hereinafter referred to as VAMC), located in Louisville, Kentucky. The Contractor s physician(s) care shall cover the range of Hematology/Oncology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Society of Hematology American Society of Hematology - Hematology.org and the American Society of Oncology (ASN) ASCO Hub - American Society of Clinical Oncology

Place of Performance: Services shall be provided onsite at the Robley Rex VAMC (also known as the Louisville VAMC) located at 800 Zorn Avenue, Louisville, KY 40206.

Pricing Instructions: The Offeror is instructed to edit the number of Sub-Line-Item Numbers (SLIN) to correspond with the number of key personnel submitted for the LIN. Other commercial health care Offerors shall identify by title/position or level of experience on the key personnel submitted. Also, renumber SLINs if adding or removing Key Personnel. For pricing purposes, the current CMS rates should be used for both inpatient and outpatient services. These rates shall not include the facility component as all services will be performed at the VA. The offeror is instructed to include all other than price and cost information supporting the proposed price as directed in Instructions to Offerors addendum to 52.212-1 and/or Section D-Contract Documents, Exhibits, or attachments, Other Than Cost and Pricing Information.

NOTE: On-call CLIN will be billed per agreed daily rate; and therefore, is not billed per CPT code. Committee/Conference attendance will be billed hourly with the maximum hours set at 132 Hours per year.

The basis of the contractor s proposed fixed percentage of Medicare rate will be used by the Government for future pricing of additional codes/procedures and for reimbursement under the contract. The basis of estimate used for your fixed price, i.e., the way that your price is calculated, shall remain the same throughout the life of the contract and may be validated by post award audits.

The Offeror shall complete the attached price schedule for on-site services at the Robley Rex, VA Medical Center. No CLINS or codes should be left blank. If for some reason a code on the schedule has expired or is for any other reason unusable, offerors shall annotate the Price per Code block with N/A or otherwise notate the replacement code.

At the end of each Specialty Area Section, the Offeror must enter a Percent of Medicare that the proposed price represents. The Percent of Medicare entry is extremely important since the award will be at that rate. Throughout contract administration, all codes reasonably considered to be included under the specialty areas will be considered in scope. The total extended price figure will be used only to determine the total estimated contract cost. The rationale for this approach is that codes and pricing change over time and an award based on Percent of Medicare Rates accounts for those changes. At the end of each CLIN, extended pricing for all cells will be totaled and entered the box entitled Est. Total Annual Price .

VA Facility Rate Should be utilized when work is performed at the VAMC because it excludes the Practice Expense (PE) RVU from the CPT Cost. The Practice Expense (PE) RVU represents the costs of maintaining a practice including rent, equipment, supplies and non-physician staff costs. The VA Facility Rate is the Work RVU and Malpractice (MP) RVU based on the appropriate locality.

The Offeror shall propose a minimum of four (4) key personnel to be credentialed and be available for scheduling to meet the requirements of the contract.

PERFORMANCE WORK STATEMENT (PWS)
GENERAL
SERVICES REQUIRED: The Contractor shall provide Onsite & On-call Board Certified /Board Eligible Hematology/Oncology Physician(s) Services in accordance with the specifications contained herein to beneficiaries of the Department of Veterans Affairs (VA) and the Robley Rex VA Medical Center in Louisville, Kentucky. The Contractor s qualified Hematology and Oncology physician(s) shall provide 24/7 call coverage; inpatient coverage (approximately 20 inpatients per month), and outpatient services, with a minimum of (3) 6-hour clinic session days per week and a maximum of (4) 6-hour clinic sessions per week as need is identified by VA; each clinic with a minimum of (25) patient slots; focusing on breast, lung and GI. Clinic session days will be negotiated and mutually agreed upon between Contractor and Medical Center, to provide coverage for other Hematology/Oncology outpatient clinics during times of staffing shortages or absences and emergencies as needed. Services will be rendered at the Robley Rex Veterans Affairs Medical Center (VAMC), 800 Zorn Avenue, Louisville, KY 40206. Contract physicians shall meet or exceed currently recognized national standards as established by The Joint Commission (TJC), and the standards of the American Hospital Association (AHA). In addition, physicians shall comply with all Veterans Health Administration s (VHA) regulations and directives. The Contractor shall provide sufficient personnel who meet all requirements herein and who can be relied on to perform in strict accordance with the contract by ensuring they are competent, qualified, and adequately trained to perform assigned duties.
The Contractor will provide 24/7 call coverage; and provide the on-call information, including contact numbers, to the VA s Medical Service Office no later than (3) working days prior to the end of each month. This information will be published on the VA s monthly call schedule.
The Contractor shall be available to provide inpatient consultation services for the diagnosis and treatment of Hematology and Oncology diseases. For inpatient services, the contractor will be available seven (7) days per week, 24 hours a day.
The Contractor shall be available to provide outpatient consultation services for the diagnosis and treatment of Hematologic and Oncology diseases. Outpatient services should be offered as needed to meet demand, with a minimum of (3) 6-hour clinic sessions day per week and a maximum of (4) 6-hour clinic sessions per week as need is identified by VA; each clinic with a minimum of (25) patient slots. It is expected that a provider will be designated for each clinic for the life of the contract to provide continuity of care, when possible, unless circumstances beyond their control necessitate a change. The goal is for all new appointments for outpatient services to be scheduled and seen within 28 days or sooner if clinically indicated. Follow-up clinic appointments are to be seen per established guidelines.
Committee/conference attendance is required in this contract, totaling 132 hours each year, to include the committees/conferences list with this contract.
The Contractor will provide both inpatient and outpatient consultation services for the diagnosis and treatment of Hematology and Oncology diseases. Contractor staff will work with VA leadership in an advisory manner on tumor board and their composition in order to improve care for our veterans.
