SOURCES SOUGHT NOTICE - VAMC WEST HAVEN
The Contractor shall deliver products and supplies to the patients homes. The contractor shall work from an established order and delivery scheduled that has set days identified, however in the case of an emergency and the patient needs supplies right away the contractor can expedite supplies overnight. The Contractor shall deliver the appropriate supplies and equipment needed for peritoneal dialysis treatments. The Contractor shall ensure that all equipment and systems provided are fully operational to manufacturer s specification and free from defect. The Contractor shall provide the Peritoneal Dialysis Systems defined by the Minimum Technical Requirements (MTRs) identified in Appendix A, Peritoneal Dialysis Systems and Peritoneal Dialysate Solution Minimum Technical Requirements (MTRs).

This announcement is seeking information from the industry, which will be used for preliminary planning purposes for an open market procurement. No proposals are being requested or accepted with this synopsis.
THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS SYNOPSIS. THIS IS STRICTLY MARKET RESEARCH.
The purpose of this synopsis is to gain knowledge of interest, capabilities, and qualifications of various members of industry, who have the capabilities to provide the required supplies to the patients home in accordance with the Statement of Work enclosed.
This contract is to be established on a firm-fixed-price.
All contractors with interest meeting the requirements identified in this notice should submit the attached area of consideration (Business Size Socioeconomic Status) with their response: The applicable NAICS and FSC/PSC code is (333310 / 6515). The eventual vendor must satisfy the requirement of supplying the items requested as specified and provide a quote at a fair and reasonable price, that also affords best value. Prior Government contracts are not required to submit a response under this Sources Sought synopsis. The intent of the Sources Sought is to identify vendors on the open market that can fulfill the requirements as stated in the Statement of Work (SOW) included below. All work for the resulting contract shall be completed by the selected vendor for each requirement in accordance with the SOW. This notice is published to conduct market research to determine if there is a sufficient number of vendors in the open market who can supply this order in aggregate, while affording competition to the fullest extent possible.


All interested firms must be registered in SAM (System for Award Management) to be eligible for award of Government contracts. Interested parties responding to this source s sought request shall submit the following information at a minimum:
(1) company name and address, point of contact with phone number, and DUNS number;
(2) documentation relating to capability of performance for servicing the equipment listed to include references with current point of contact and phone number);
(3) socio-economic status to identify business class as being a large business, SDVOSB, VOSB, small business, Woman Owned, etc.
(4) completed Area of Consideration form; and
(5) any other pertinent company documentation, prior to submission of offers to a solicitation.

Any SD/VOSB firms responding to this Sources Sought Notice must also provide proof of registration in the SBA VetCert/VetBiz, prior to submission of offers to a solicitation.

RESULTS INFORMATION: Respondents will not be notified of the results and it is at the discretion of the government to utilize results. Firms responding to this Sources Sought announcement, who fail to provide ALL of the required information requested, will not be used to help the Government make the acquisition decision, which is the intent of this sources sought announcement.
