Is your firm a local business?
Local business is a business with a permanent place of business with a Brooksville address and is the address from which the vendor does business with full-time employees within the city limits for a minimum of one year prior to the date of issuance of a bid or proposal solicitation. Merely having a mail drop within the City of Brooksville is not sufficient to establish a business as being a local business.
Is your firm a Local County Business?
Local County Business is a business with a permanent place of business with a Hernando County address and is the address from which the vendor does business with full-time employees within the county limits for a minimum of one year prior to the date of issuance of a bid or proposal solicitation. Merely having a mail drop within Hernando County is not sufficient to establish a business as being a local county business.
How many years has your firm been in business as a contractor?
List up to three (3) projects of this nature that you have completed in the last three
(3) years, and give the name, address and telephone number of a reference from each. Also give the completion date, the original contract bid price and the completed cost of each project listed.
List projects presently under construction by your firm, the dollar volume of the contract and the percentage completion of the contract.
Has your firm ever failed to complete the work awarded? If so, state when, where and why.
Is your firm presently or has your firm ever been a party defendant in a lawsuit commenced against your firm alleging failure to properly complete work in accordance with the contract for same? If so, give details.
Has your firm received two (2) final determinations within any consecutive six- year period, the second final determination occurring within the past five (5) years, that your firm willfully failed to pay the prevailing rate of wages or to provide supplements in accordance with Article 8 of the Labor Law? If so, give details.
