The Maryland Department of Health  (MDH or the Department), Center for Chronic Disease Prevention and Control (CCDPC) is issuing this Request for Proposals (RFP) to provide an implementation progress review report of the Maryland State Plan to Address Alzheimer’s Disease and Related Dementias: 2022-2026 (the Plan), lead a strategic planning and engagement process to update the Plan for 2027-2031, and produce web-ready, and physical copies of the 2027-2031 Plan, and an accompanied presentation slide deck. The 2027-2031 Plan will be distributed to various stakeholders in Maryland and guide interventions to address ADRD burden across the state.
