Description:
The County of Los Angeles Justice, Care and Opportunities Department (JCOD) is releasing this Work Order Solicitation (WOS) #JC-030526-J1 to issue a Work Order under the Justice Supportive Services Master Agreement (JSSMA) to a Qualified Contractor to provide JCOD Care Management (JCM) Program Including Enhanced Care Management (ECM) Services to adults and transition-age youth who are justice-involved or justice-impacted and reside in Los Angeles County. The purpose of this WOS is to identify and contract with organizations that demonstrate the capacity to design and deliver comprehensive, person-centered, trauma-informed, and culturally responsive care management services that address individuals’ medical, behavioral health, housing, employment, and social support needs. The JCM program is designed to improve health, housing, and social outcomes through holistic, community-based, and relationship-driven approaches that address both the clinical and non-clinical needs of program participants.
