1. Project Overview
Lake County Behavioral Health Services (LCBHS) is pleased to announce the availability of
Behavioral Health Services Act (BHSA) Behavioral Health Services and Supports (BHSS) funding to
support planning, start-up, implementation, operations, and non-billable components of a
member-centered, recovery-oriented Clubhouse model for Transition Age Youth (TAY) for up to
three years. Funding is anticipated at approximately $200,000 in Year 1, $150,000 in Year 2, and
$100,000 in Year 3, for a total anticipated award of up to $450,000, subject to County approval,
contract execution, and available funding. This funding is intended to support readiness for Medi-
Cal reimbursable Clubhouse Services under BH-CONNECT; it does not guarantee Medi-Cal
reimbursement, billing approval, or future funding.
Recent History: Lake County Behavioral Health Services, with community cooperation, has
historically supported a Transitional Age Youth (TAY) drop-in center and peer support services.
The center has provided peer support, guidance, resources, linkage to services, and support for
youth and young adults ages 16-24.
Current Need: There is an ongoing need to assist vulnerable TAY in Lake County, including youth
and young adults ages 16-24 who are at risk of significant mental health issues, homelessness,
juvenile justice involvement, and school failure. The program should provide a safe and
welcoming place for outreach, drop-in engagement, resources, groups, recovery-oriented
activities, and connection to services. Within this broader population, only some participants may
meet Medi-Cal eligibility, medical necessity, and other applicable criteria for future Medi-Cal
reimbursable Clubhouse Services.
Funding Purpose: With the transition from the Mental Health Services Act to the Behavioral Health
Services Act, funding priorities have shifted. LCBHS is offering BHSA BHSS funding to support
planning, start-up, implementation, operations, and non-billable activities needed to establish a
TAY Clubhouse model. The contractor will be expected to work toward readiness for Medi-Cal
reimbursable Clubhouse Services, subject to DHCS requirements, County BH-CONNECT
implementation decisions, Clubhouse fidelity standards, provider contracting, documentation
requirements, and applicable Medi-Cal billing rules. Funding is anticipated to decrease over the
three-year period as the program moves toward operational readiness and sustainability, subject
to County decisions and available funding.
Clubhouse Implementation: Applicants should be prepared to implement a member-centered,
recovery-oriented Clubhouse model that is inclusive of peers and individuals with lived
experience in program design and service delivery while remaining consistent with Clubhouse
fidelity expectations. Applicants should describe their anticipated timeline and strategy for
achieving Clubhouse fidelity/readiness milestones and readiness for Medi-Cal reimbursable
Clubhouse Services under BH-CONNECT, subject to all applicable requirements.
Program Parameters: The County expects the TAY Clubhouse to operate at least four (4) days per
week and at least twenty (20) hours per week, unless otherwise negotiated in the final contract.
Evening and weekend services are encouraged. Proposals should include anticipated participant
capacity, expected unduplicated participants served annually, and average daily or weekly
participation. Services must remain available to eligible Lake County residents at no cost during
the award period.

2. Eligible Applicants
Eligible applicants may include:
A unit of local government.
A registered 501(c)(3) nonprofit organization.
A state or federally recognized tribal entity.
Partnerships and subcontracting arrangements are acceptable; however, one lead applicant
must be identified and will be responsible for contract compliance, fiscal accountability,
reporting, and performance.
3. Eligible Activities
Award funds must be used for operating expenses that support planning, start-up,
implementation, operations, and non-billable components of the TAY Clubhouse model.
Allowable costs may include staffing, food, utilities, supplies, insurance, minor equipment
necessary for operations, professional development, outreach, and other reasonable costs
associated with operating a Clubhouse center, as described in the budget narrative. Construction
and capital facility costs are not allowable under this RFP unless expressly approved in writing by
the County prior to contract execution.
The contractor will be expected to provide outreach and engagement to TAY through culturally
responsive resources, groups, activities, and appropriate services using a member-centered,
recovery-oriented Clubhouse model. Preference will be given to applicants that demonstrate
meaningful integration of peers and individuals with lived experience in service delivery and
program design, consistent with Clubhouse fidelity expectations.
Applicants must describe how proposed activities and services will promote recovery, wellness,
permanent housing linkages, education, employment, community connection, and readiness for
Clubhouse fidelity standards. All activities must be conducted within Lake County and serve
eligible Lake County residents at no cost during the award period.
4. Program Requirements
The selected contractor will be responsible for obtaining and maintaining all required insurance,
fire clearance, occupancy approval, city or county inspections if applicable, and any other
approvals necessary for the proposed program site and activities.

The contractor must maintain participant records and comply with all applicable confidentiality,
HIPAA, County documentation, and reporting requirements. Medi-Cal documentation
requirements apply if and when services are delivered as Medi-Cal reimbursable Clubhouse
Services.
At a minimum, participant data should include:
Number of unduplicated people served.
Demographic information, including age, sex, sexual orientation, education status, veteran
status, and disability status, to the extent collected consistent with applicable law and
participant privacy.
Services, activities, referrals, linkages, and outcomes.
Deliverables and Reporting Expectations
Operate the TAY Clubhouse at least four (4) days per week and at least twenty (20) hours per
week, unless otherwise negotiated in the final contract.
Maintain participant capacity targets, including unduplicated participants served annually and
average daily or weekly participation.
Conduct outreach and engagement to TAY-serving partners, including County Behavioral
Health, schools, youth-serving organizations, probation/justice partners, housing and
homelessness providers, tribal partners, and community-based organizations, as applicable.
Develop and report progress on Clubhouse fidelity/readiness milestones, including
implementation planning, staff training, technical assistance participation, documentation
procedures, and readiness for provider contracting and billing rules if applicable.
Maintain documentation sufficient to support services provided, referrals/linkages made,
participant demographics, outcomes, and fiscal accountability.
Submit reports in a format and frequency required by LCBHS, including annual reports and
any additional reports needed for audits, monitoring, BHSA reporting, or other applicable
requirements.
Annual Reports
The contractor will be required to submit annual reports to LCBHS that include, at a minimum:
Expenditures made within the fiscal year.
Activities provided within the fiscal year.
Operating hours and schedule offered.
Unduplicated participants served, average daily or weekly participation, and participant
capacity utilization.
Outreach activities, referral sources, and referrals/linkages made.
Progress toward Clubhouse fidelity/readiness milestones.
Performance outcomes and data quality review.
LCBHS may request additional information as needed to meet applicable reporting, monitoring,
audit, BHSA, or County requirements.
LCBHS will monitor contractor expenditures and activities as LCBHS deems necessary, including
review prior to the obligation or payment of funds to ensure compliance with program

requirements. LCBHS may stop payment, request repayment, or pursue any other remedy
available by law for failure to comply with program requirements. After a contract has expired, any
funds not expended for eligible activities shall revert and must be remitted back to LCBHS.
Housing and Homelessness Coordination
If the contractor provides housing navigation, homelessness-related services, or serves as a
formal access point for the Lake County Continuum of Care, the contractor may be required to
participate in the Coordinated Entry System (CES) and use the Homeless Management
Information System (HMIS), as applicable. These requirements will be negotiated and included in
the final contract only if applicable to the approved scope of work.
If applicable, expectations may include:
Accept referrals and prioritize clients in alignment with CES policies.
Participate in case conferencing and housing prioritization processes.
Enter and maintain accurate, timely client-level data in HMIS.
Track and report required housing and homelessness-related performance metrics.
Technical assistance may be provided through LCBHS and the Lake County Continuum of Care,
as applicable.
5. Conflict of Interest
The applicant must disclose, as part of the proposal, any actual, potential, or perceived conflicts
of interest that may result from the award of the contract or the services provided under the
contract.
Except as otherwise disclosed in the proposal, the applicant affirms that, to the best of its
knowledge, there is no actual or potential conflict between the applicant, the applicant’s project
manager(s), or any related business or financial interests and the services to be provided under
the contract. If a conflict arises or becomes known during the RFP process or contract term, the
applicant must immediately notify LCBHS. All conflicts must be resolved to LCBHS’ satisfaction,
or the applicant may be disqualified or the contract may be terminated.
For purposes of this RFP, conflicts of interest include, but are not limited to:
Giving or offering a gratuity, kickback, money, gift, or anything of value to an LCBHS official,
officer, or employee with the intent of receiving favorable treatment regarding a contract.
Having or acquiring any contractual, financial, business, or other direct or indirect interest that
would conflict with the applicant’s performance of duties and responsibilities under the
contract or create the appearance of impropriety concerning the award or performance of the
contract.
Possessing or accepting anything of value during the RFP process or contract term based on
an understanding that the actions of the applicant or its affiliates on behalf of LCBHS will be
influenced.
