The Illinois Department of Human Services, Bureau of Family Nutrition is seeking a vendor to provide infant formula rebates. Currently the Federal program WIC "receives" a rebate for infant formula from Mead Johnson. The contract will be awarded based on the "lowest annual net formula cost" from a single provider of infant formula. The Department contracts with one manufacturer whose iron-fortified milk based 13-ounce liquid concentrate, 12-12.9 ounce powdered, and 32-ounce ready-to-use infant formulas will be the basis for determining the lowest monthly net formula cost. The Contractor must pay a rebate to the Department for a fixed amount per container of any formula in the product line including any new infant formula products introduced during the contract term that the State agency chooses to issue which meet the definition of "infant formula." This will include, but not be limited to, soy-based and lactose free, formula purchased through the Illinois WIC Program. The three-year initial term shall 2/1/2026 to 1/31/2029.
