Description:
Private Residence-Not Commercial Facility or Publicly Funded Group Home-Contractor must be enrolled with SC Medicaid as a BHDD-OIDD waiver service provider for Environmental Modifications. Only bids from qualified enrolled providers will be accepted. Necessary Modifications to improve accessibility and safety of individual while completing ADLs in bathroom. Remove current shower setup and install new shower. Install new low threshold shower, with grab bars, and fold down seat.
