Description
STATEMENT OF WORK (SOW)
1. Contract Title.

Microbiological media purchase order for Clinical Microbiology laboratory.
2. Background.
A purchase order is requested to order specific products throughout the year necessary to perform microbiology lab tests.
3. Scope.

FY26 request for microbiology testing media for Seattle Division Microbiology Department. These products are required for the identification of pathogenic organisms. This includes products formatted to maintain a long shelf-life, such as the BactiDrop Oxidase, Indole and Acridine Orange that are used daily and are critical for organism identification, as well as perishable selective agar, biochemical test kits and other required products such as ATCC organisms, mandated by the College of American Pathologists (CAP), as part of our Quality Control for media. Products are ordered monthly for weekly delivery as noted below or as needed, and these supplies enable microbiologists to identify diseases in veteran patients.

4. Specific Tasks.

The vendor will deliver the kits and reagents associated with this contract on the date specified or within a reasonable window of time after ordering has ensued. The vendor will adhere to the priced schedule submitted with the quote of these materials, of which price of shipping is included in the cost of material. The vendor will provide SDS sheets for all media upon request. Times and dates of delivery to be determined by the PSHCS Microbiology Department. At the beginning of the month, weekly scheduled shipments of products with the expected delivery date and product counts will be submitted. See Appendix A for example of weekly product delivery.

The vendor will be able to provide all products listed in the Appendix B

The vendor will supply acknowledgement of product requests as they are submitted by the PSHCS Microbiology Department and provide tracking information for shipments due to the perishable nature of required goods. The vendor will monitor any back orders or shipping delays with prompt communication of any changes.

4.1 Task 1 - Enterprise Management Controls.

Subtask 1 - Integration Management Control Planning.
Microbiology employee or supervisor enters request for product from vendor.
Vendor ships out product, referencing PO# for payment.
Vendor supplies shipping information or notifications of product delays.
Microbiology employee receives product, inspects for discrepancies.
Vendor bills for product, Purchasing Agent checks for delivery completeness from end-user.
Purchasing agent completes receiving protocol in computer.
Microbiology Department uses product to perform diagnostic testing for veteran patients.
4.2 Task 2
Subtask 2 - Contract Management.
Products are ordered at Seattle by the Microbiology Department in coordination with Administrative Specialist COR David Travelstead. The order is sent to the vendor via email or website, referencing the correct purchase order given from Fiscal and Contracting. Any delivery delays are sent via email to the purchaser in the Microbiology Department, who coordinates with vendor. Vendor sends invoices for payment to the VA Accounting Department in Austin, TX. Austin Automated Payment System posts the invoices for the laboratory to review, and then the invoices will be certified after confirmation of received items.
4.3 Task 3
Subtask 3 Prices and Terms.
Seattle VAMC will provide an estimated volume by test as reflected in Attachment B. Pricing and volume are based on the AVERAGE yearly volume of products. The Government estimates the volumes as listed in Attachment B but does not guarantee volumes as listed; they are estimates ONLY. The Government reserves the right to modify the contract to update product numbers or items that are discontinued and replaced with equivalent products.

5. Performance Monitoring
The Microbiology Department must adhere to specific standards for all laboratory consumables. These standards are applied to the products ordered from this vendor. COTR and/or any Microbiology employee are responsible for monitoring all products for quality.
6. Security Requirements
There are no security related components to this contract request. The vendor will not have access to protected patient information. Protected patient information will not be input into the vendor computer and the vendor computer associated with the testing instrumentation will not be interfaced with any VA networks
7. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI).

There is no government furnished equipment associated with this contract.

8. Other Pertinent Information or Special Considerations.

Identification of Possible Follow-on Work. None

Identification of Potential Conflicts of Interest (COI). None

Identification of Non-Disclosure Requirements.
Contractor will not have access to any veteran names or diagnosis.
This is a consumable item, not specific to any one patient.

Packaging, Packing and Shipping Instructions.
To be delivered on Tuesdays for weekly shipment requests and as soon as available for individual product requests. The products are temperature liable and requires immediate refrigeration upon delivery. The cost of shipping for next-day delivery for all perishable goods must be included in the individual item cost.

Inspection and Acceptance Criteria.
Microbiology POC, Supervisor and any other Microbiology employees have specific lab standards to maintain for all products under their care.
9. Risk Control
Microbiology is responsible and will accept any risks associated with product, notified by vendor.
10. Place of Performance.

Specific area to receive and work with product from vendor is the Microbiology lab at the Seattle Division of VA Puget Sound Medical Center.
