Sources Sought Notice (SSN) Department of Veterans Affairs, Veterans Health Administration, Network Contracting Office 16 (NCO 16) This SSN is for market research and planning purposes only, and shall not be considered an Invitation for Bid (IFB), Request for Task Execution Plan, Request for Quotation (RFQ), or a Request for Proposal (RFP). Additionally, there is no obligation on the part of the Government to acquire any products or services described in this SSN. Your response to this SSN will be treated only as information for the Government to consider. You will not be entitled to payment for direct or indirect costs that you incur in responding to this SSN. This request does not constitute a solicitation for proposals or the authority to enter into negotiations to award a contract. No funds have been authorized, appropriated or received for this effort. Interested parties are responsible for adequately marking proprietary, restricted or competition sensitive information contained in their response. The Government does not intend to pay for the information submitted in response to this SSN. The North American Industry Classification System (NAICS) for this requirement is 339112 with a minimum business size of 1,000 employees and Product Service Code (PSC) of 6515. Purpose The overall purpose is to deliver, (1) WavelinQ R/F Generator (brand equivalent or better) and (1) WavelinQ Arm Board (brand equivalent or better) to CAVHS Imaging Service. This product shall include a minimum one (1) year warranty. Scope The WavelinQ RF Generator and Arm Board delivered via this acquisition shall safely and economically allow percutaneous endovascular creation of AV fistulae in an outpatient, non-OR setting, by placing a catheter in the forearm vein and another catheter in a parallel forearm artery and using magnets to join the adjacent vessels followed by a brief radiofrequency pulse to create an anastomosis between the artery and vein. The Contractor shall provide at no additional cost, sufficient training to ensure safe and efficient use of the RF Generator and Arm Board to CAVHS IR personnel. Delivery will be made to Central Arkansas Veterans Healthcare System, 2200 Fort Roots Drive Bldg 182 (Supply Warehouse), North Little Rock, AR 72114 Operator Training Contractor shall provide on-site training of the equipment to the Users. Scheduling of operator training shall be coordinated with the CAVHS POC. Warranty The contractor shall provide a one-year manufacturer s warranty on all parts. The warranty shall include all shipping costs associated with any warranty repair.
