Project Description: The City of Kansas City, Missouri (“City”), through the Health Department (HD), is seeking proposals from qualified community-based organizations that serve targeted communities and populations where chronic disease rates are highest, to implement initiatives that support community members in altering their behaviors to lower the risks of chronic disease. We intend to award 3 contracts of up to $40,000 to an organization from each of the 3 Health Prosperity Zones (North, Central and South) with preference given to organizations whose proposal targets residents within those zones with the lowest health opportunities.
