Description
The purpose of this Request for Proposal (RFP) # PHTHOET27001 is to solicit applications that will enable the Iowa Department of Health and Human Services (referred to as Agency) to select the most qualified applicants to initiate or expand Mobile Integrated Health (MIH) services within defined rural areas across Iowa. The purpose of developing Mobile Integrated Health projects is to initiate or expand treat-in-place care, telehealth-enabled clinical assessments, chronic disease management, post-discharge follow-up, prenatal and postpartum home-based support, medication reconciliation, and other community-based clinical interventions for Rural Iowans.
