Notice is hereby given that the City of Coweta will accept sealed proposals from interested and qualified bidders for furnishing a CUSTOM CAB FIRE RESCUE TRUCK. Proposals will be accepted at Coweta City Hall, 310 South Broadway, Coweta, OK, 74429, until 1:00 p.m., Wednesday, February 5, 2025, and at that time and place, the proposals will be publicly opened and read.
Bidders will submit their bid on the provided form in a sealed envelope, marked on the outside as follows: “SEALED PROPOSAL TO BE OPENED AT 1:00 P.M. on FEBRUARY 5, 2025.
CUSTOM CAB FIRE RESCUE TRUCK
, Attention city clerk.” to: City of Coweta, P.O. Box 850, 310 S. Broadway, Coweta, OK, 74429-0850.
All bids submitted must contain the following:
1. PROPOSAL FORM
: Complete and submit the PROPOSAL FORM found in ATTACHMENT 1.
2. SIGNATURE PAGE
: Complete and submit the SIGNATURE PAGE found in ATTACHMENT 2. The Signature Page must be signed by a person, or persons, authorized to bind the entity, or entities, submitting the proposal. Proposals signed by a person other than a representative of the company shall be accompanied by evidence of authority.
3. REFERENCES:
Complete and submit the REFERENCES page found in ATTACHMENT 3. Include the names and address of at least three entities that have purchased the products being proposed.
4. NON-COLLUSION AFFIDAVIT:
Complete the NON-COLLUSION AFFIDAVIT on ATTACHMENT 4.
5. ACKNOWLEDGMENT OF ADDENDUMS
: Contractors must acknowledge receipt of any addendums to the RFP by signing and returning the Addendum form with the Proposal, by identifying the addendum number and date in the space provided for this purpose on the addendum form to be provided in the event an addendum is issued.
