ELEVATOR ANNUAL INSPECTION

SCOPE OF WORK
Vendor will furnish inspectors Certified as Qualified Elevator Inspector (QEI) by the accredited certifying organization NAESA Int’l or QEI Training Fund to perform and report Elevator Periodic Inspections per the jurisdiction's requirements regulations, ASMEA17.1. Jurisdictional requirements and ASME A 17 .1 at the time of installation or alteration based on Adopted Standards.

This will include:
Coordination and performance of Periodic Inspections once a year based on due date.
Completion of the Department of Labor Annual Conveyance Safety Inspection Forms.

Forms will provide any violations of code along with a:

1. Description of the violation
2. And the code reference for the violations

The forms must be submitted to the Department of Labor Elevator Division and contact information must be provided to the building manager and Diversified Elevator.

The Qualified Vendor must have:
A minimum of 5 QEI inspectors and must live in Alabama.
A minimum of 5 million dollars general liability insurance.
