6520--Dental Clinic Activation Equipment

The purpose of this amendment is to extend the solicitation due date and time and revise ADDENDUM to FAR 52.212-1 INSTRUCTIONS TO OFFERORS.

The Bay Pines VA Clinic, Building 112, 10000 Bay Pines Blvd. Bay Pines, FL 33744, has a requirement for Brand Name or Equal Dental Clinic Activation Equipment. This requirement is set aside for Service- Disabled Veteran Owned Small Businesses Only. All vendors must be properly registered in the System Award Management database (SAM), SBA Veteran Small Business Certification (VetCert), and SBA Dynamic Small Business Search (DSBS). This solicitation will allow for vendors to offer BRAND NAME OR EQUAL products. All items shall be new, and not refurbished or recycled.

Price quotes shall include the Unique Entity Identifier (UEI) Number, Veteran Small Business Certification (VetCert), Small Business Status, an authorized distributor letter on the manufacturer’s letterhead if applicable, a copy of a price list or any other supporting information for the price submitted, and a COMPLETED FAR 52.225-1 BUY AMERICAN – SUPPLIES (If applicable) and VAAR 852.219-76 VA NOTICE OF LIMITATIONS ON SUBCONTRACTING – CERTIFICATE OF COMPLIANCE FOR SUPPLIES AND PRODUCTS (DEVIATION).
