This addendum forms part of the contract document and forms the original bid documents. The

following revision, clarification, deletion, and/or additions shall be made to the RFP documents

for RFP No. R25-26-9. All other requirements of the contract documents remain the same. It is

mandatory to acknowledge receipt of this addendum by inserting its number into the

location provided in the RFP Document.

This addendum consists of the following changes to the bid document:

I. Clarification

PART I – CLARIFICATION

A. The requirements for Sexual Abuse and Molestation Coverage are clarified as follows:

• $3,000,000 per wrongful act

• $6,000,000 aggregate
