Conducting Procurement Unit: The Utah Department of Health and Human Services
The purpose of this RFP is to enter into a contract to conduct consumer satisfaction surveys for health plan enrollees, dental plan enrollees, and medical home patients.
It is anticipated that this RFP will result in a single contract award to the responsive and responsible vendor with the highest score justified by the procurement code.
The methodology or formula that will be used to determine the number of contract awards is: Highest scoring responsive and responsible offeror.
Length of Contract: 5 years
Vendor must guarantee its pricing for one year.
