1. PURPOSE AND OVERVIEW
Introduction
The New Hampshire Department of Health and Human Services, Division for Behavioral Health
("Department"), on behalf of the Opioid Abatement Advisory Commission, is seeking responses
to this Request for Grant Applications ("RGA" or "solicitation") from qualified Applicants to
develop and implement initiatives and services in New Hampshire aimed at mitigating the
impacts of the opioid crisis.
Qualified Applicants include the following:
* Any non-profit or charitable organization registered and in good standing with the New
Hampshire attorney general's charitable trusts unit;
* A state agency, board, or commission; or
* Governmental entities, including school districts, towns, cities, and counties within the state
of New Hampshire.
Organizations that are:
* Not under agreement with the Department for Opioid Abatement Commission funded
projects will be given priority under this RGA; and
o Currently under agreement with the Department for Opioid Abatement Commission funded
projects may not submit applications for duplicative projects.
The Department anticipates awarding multiple Grant Agreements for projects, initiatives and/or
activities ("Project") that meet the objectives and requirements in this solicitation.

From the date of release of this solicitation until an award is made and announced regarding the
selection of an Applicant, all communication with personnel employed by or under contract with the
Department regarding this solicitation is prohibited unless first approved by the Point of Contact
listed above. Department employees have been directed not to hold conferences and/or discussions
concerning this solicitation with any potential grantee during the selection process, unless otherwise
authorized by the Point of Contact. Applicants may be disqualified for violating this restriction on

Background
New Hampshire Department of Health and Human Services, Division for
Behavioral Health
The Department is dedicated to providing a comprehensive and coordinated system of
health and human services that promotes and protects the health, safety and well-being of
the citizens of New Hampshire. Such services are directed at supporting families,
strengthening communities, and developing the independence and self-sufficiency of New
Hampshire citizens to the extent possible. Behavioral health of New Hampshire citizens is
central to this mission.
New Hampshire Opioid Abatement Trust Fund & Advisory Commission
The Opioid Abatement Trust Fund ("Trust Fund") was established by RSA 126-A:83. In
accordance with RSA 126-A:84, 15% of proceeds received by the State from all consumer
protection settlements or judgments against opioid manufacturers or distributors are
distributed to qualifying subdivisions, and remaining funds are deposited into the Trust Fund
to be distributed for approved uses to qualifying entities.
The New Hampshire Opioid Abatement Advisory Commission (OAC) was established by
RSA 126-A:83-86 and New Hampshire Administrative Rule Chapter He-C 1000 OPIOID
ABATEMENT TRUST FUND AND ADVISORY COMMISSION. The OAC consults with the
Department and the New Hampshire Department of Justice regarding the proper
administration and management of the Trust Fund.

Objective
The Department is seeking qualified Applicants to develop and implement initiatives and
services aimed at mitigating the impacts of the opioid crisis.
Covered Populations
Populations served may include, but are not limited to, individuals with, or at risk of
developing, opioid use disorders (OUDs) and any co-occurring substance use disorder or
mental health (SUD/MH) issues.
Grant Agreement Requirements
Applicants must be an eligible entity as described in Section 1.1.
Applicants must develop and implement initiatives and services aimed at mitigating
the impacts of the opioid crisis that are aligned with one or more of the qualifying
criteria established under RSA 126-A:86 and the OAC Guiding Principles,
according to the following Priorities:
Priority 1: Expand prevention and family strengthening services for older
adults, including, but not limited to:
Supporting evidence-based and/or evidence-informed
primary, secondary, and tertiary prevention programs and
services, including efforts to:
Promote healthy lifestyles, reduce isolation,
build skills and resilience and facilitate
community-based prevention efforts
particularly for older adults; and
Providing support for public and non-public
school programs and services for students
with OUD and any cooccurring SUD/MH
issues or who have been affected by OUD
and any co-occurring SUD/MH issues within
their family.
Supporting family strengthening programs for children,
youth and families impacted by OUD/SUD including kinship
caregivers and foster parents.
Priority 2: Expand access to appropriate treatment for opioid use disorder,
including residential care and access to methadone in underserved areas
of the state.
Priority 3: Provide access to housing to support treatment, recovery, and
stability.
Qualifying Projects under this Priority may include capital
purchases and/or renovations of facilities to provide access
to housing to support treatment, recovery, and stability.
Qualifying Projects do not include ongoing services.
Priority 4: Provide or support transportation to treatment or recovery
programs.

Qualifying Projects under this Priority may include capital
purchases to provide or support transportation to treatment
or recovery programs. Qualifying Projects do not include
ongoing services.
Priority 5: Support services for individuals who have responded well to
opioid therapy but have experienced non-consensual dose reduction or
discontinuation, and support services that increase access to
comprehensive, integrative pain management as an alternative to opioid
therapy; and
Priority 6: Support first responders through training, resources, and safety
focused initiatives.
Applicants must provide a Project Plan that outlines in detail each proposed
Project, strategy, and/or initiative, which must meet the objective of mitigating
impacts of the opioid crisis and must include, but is not limited to:
Program, strategy, or initiative title(s).
Description(s) of the program, strategy, or initiative.
Purpose and Objective.
Funding requested.
Implementation timeline.
Measurable outcomes.
Reporting Requirements
Selected Applicants must submit a report to the Department for
distribution to the Commission with content identified by the
Commission, and in a format as required by the Commission, by the due
date specified in the resulting Grant Award.
Additional Requirements (as applicable based on the selected Applicant's Project)
Background Checks
Prior to permitting any individual to provide services under this
Agreement, selected Applicants may be required to ensure that said
individual has undergone:
A criminal background check, at the selected Vendor's
expense, and has no convictions for crimes that represent
evidence of behavior that could endanger individuals served
under the resulting agreement;
A name search of the Department's Bureau of Adult and
Aging Services (BAAS) State Registry, pursuant to RSA
161-F:49, with results indicating no evidence of behavior
that could endanger individuals served under the resulting
agreement; and
A name search of the Department's Division for Children,
Youth and Families (DCYF) Central Registry pursuant to
RSA 169-C:35, with results indicating no evidence of

behavior that could endanger individuals served under the
resulting agreement.
Confidential Data
If the Project in the resulting Grant Agreement involves the collection,
exchange, or storage of any confidential data, the selected Applicant
must meet all information security and privacy requirements as set by
the Department and in accordance with Appendix A, Standard Exhibits,
Exhibit D: DHHS Information Security Requirements.
Selected Applicants must promote and maintain an awareness of the
importance of securing the Department's information among their End
Users. End Users must not be permitted to handle, access, view, store
or discuss Confidential Data until an attestation is executed by the
individual End User and received by the Selected Applicant certifying
that the individual End User associated with fulfilling the obligations of
the Grant Agreement is, based on criteria provided herein, eligible to
participate in work associated with the Grant Agreement.
Selected Applicants must ensure any staff and/or volunteers involved in
delivering services through the resulting Grant Agreements sign an
attestation agreeing to access, view, store, and discuss Confidential
Data in accordance with federal and state laws and regulations, and
Appendix A, Standard Exhibits, Exhibit D: DHHS Information Security
Requirements. Selected Applicants must ensure said individuals have a
justifiable business need to access confidential data. Selected
Applicants must provide attestations upon Department request.
Upon request, Selected Applicant must allow and assist the Department
in conducting a Privacy Impact Assessment (PIA) of its
system(s)/application(s)/web portal(s)/website(s) or Department
system(s)/application(s)/web portal(s)/ website(s) hosted by Selected
Applicant if Personally Identifiable Information (PII) is collected, used,
accessed, shared, or stored. To conduct the PIA, Selected Applicant
must provide the Department access to applicable systems and
documentation sufficient to allow the Department to assess, at minimum,
the following:
How PII is gathered and stored;
Who will have access to PII;
How PII will be used in the system;
How individual consent will be achieved and revoked;
and
Privacy practices.
1.8.5. The Department may conduct follow-up PIAs in the event of:
Significant process changes; and/or
New technologies impacting the collection, processing
or storage of PII.

Applicants are required to consider the need for language services for
individuals with Limited English Proficiency as well as other
communication needs, served or likely to be encountered in the eligible
service population, both in developing their budgets and in conducting
their programs and activities.
Applicants are required to complete Appendix B, Culturally and
Linguistically Appropriate Services (CLAS) Requirements, as part of
their solicitation response. This is in accordance with Federal civil rights
laws and intended to help inform Applicants' program design, which in
turn, will allow Applicants to put forth the best possible solicitation
response.
If applicable based on the selected Applicant's Project, selected
Applicants may be:
Required to submit a detailed description of the
language assistance services they will provide to LEP
persons to ensure meaningful access to their programs
and/or services, within ten (10) days of the date the
Grant Agreement is approved by Governor and Council;
and
Monitored on their Federal civil rights compliance using
the Federal Civil Rights Compliance Checklist, as made
available by the Department.
2. Funding and Award Process
Mandatory Questions
Applicants must provide a written response to each of the mandatory questions
below. Applicant responses to each question must not be longer than five (5)
pages in length. Applicants must submit a separate proposal for each proposed
Priority.
Project Need - Describe in narrative form the need for the Project. Include the total
funding amount requested, and a description of any current grants or financial
assistance, apart from the trust funds, the applicant is currently receiving from other
funding sources that relate to the proposed project and the applicant's request for
funding. Please explain what will happen if your organization is not funded.
Project Plan - Provide a Project Plan for the proposed Project or necessary
activities, that align with the objectives of this RGA. Include any existing
infrastructure or interfaces and how they may be leveraged towards meeting the
objectives of this project. The Project Plan must include:
Program, strategy, or initiative title(s);
Description(s) of the program, strategy, or initiative;
Purpose and Objective;
Implementation timeline; and
Measurable outcomes.

Collaboration - Describe, in narrative form, any proposed collaborations with other
providers. Include the role of each collaborating partner and how you will handle
coordination.
Ability - Provide a description of your organization's ability to provide opioid
abatement services and/or programs, including past achievements and evidence of
impact.
Funding Request - Provide a budget sheet with proposed Project costs for each
State Fiscal Year (July 1 - June 30) utilizing Appendix D - Budget Sheet, including
a budget narrative explaining the specific line-item costs and the direct relationship
to meeting the objectives of the project. Include any supporting documentation for
the funding being requested under this RGA, which may include, but is not limited
to:
An itemized breakdown of each cost.
If applicable, justifications for hiring a consultant or contractor and a copy of
each agreement.
Estimates or quotes received from licensed or certified professionals,
including, but not limited to:
* Legal business name of professional business.
* Purchases.
* Labor expenses.
* Parts and materials.
* Required inspections expenses, if applicable.
Receipts and/or invoices from a professional business for services rendered
relating to the objectives of this RGA with dates of services between July 1,
2026 (estimated and contingent upon Governor and Executive Council
approval) and June 30, 2028.
