Health Risk Screening Tool (HRST) Screening Services

The Department of Developmental Disabilities (DODD) goal is to identify health risks early, support effective planning, and improve health and safety outcomes for the people with I/DD receiving services at DODD’s developmental centers. DODD has identified the Health Risk Screening Tool (HRST) as the desired method to be used to detect health risks and destabilization early in individuals with intellectual and developmental disabilities (I/DD) and therefore assist those responsible for support and planning the needs of these individuals.

DODD is requesting a quote for the provision of initial health screenings in FY26 and annual / secondary health screenings in FY27 using the Health Risk Screening Tool (HRST) for approximately 650 people at the agency’s developmental centers. Contractor will be responsible for providing IntellectAbility trained raters to perform HRST screenings.

Bidder must also be authorized to provide quote for annual licenses of the HRST web-based application, for FY26 and FY27, for each individual DODD identifies to receive an HRST screening. DODD staff would have view-only access to the application, to enable them to generate both individual and aggregate reports and to support continued use of the service and training considerations generated by the tool.

Bidders must complete the “Item Grid” in OhioBuys and attach an electronic copy of their quote within the solicitation in OhioBuys.

The attached PDF fillable forms, Standard Affirmation and Disclosure Form (EO 2019-12D & 2022-02D) and Contractor Information Form are mandatory requirements for completion and submission for the bid to be considered, for evaluation. The attached PDF fillable form, Contractor Certification Statements, is required if a bidder is eligible for a preference in relation to the goods and services to be provided under this purchase in order to be considered as part of the evaluation of their bid.

Bids will be evaluated, and an award will be made to the lowest responsive and responsible bidder as determined by the agency. Bids will only be accepted for The Aegis System training and certification, no alternative training / certifications will be accepted.
