Scope of Services
The Orlando Utilities Commission (OUC) is requesting proposals from qualified suppliers of Occupational Medical Services. The purpose of this solicitation is to select one (1) or more Suppliers capable of delivering a range of services, including pre-employment post-offer examinations, general medical examinations, fitness-for-duty evaluations, exposure response, random drug and alcohol screenings, and Medical Director consultation to support OUC’s Occupational Health and Safety Programs in accordance with Attachment A: Scope of Work and Requirements.

9.        If we offer a comprehensive exposure program, can we provide additional detail on the program and costs. The Price Schedule only allows for an individual cost which does not support how we would need to include pricing.
10.        Attachment A – Detailed Scope – Section E - Does OUC still utilize an HRA for their annual wellness program offering? If so, can you please define the requirements for a Health Risk Assessment outlined on page 5 in Attachment A.
11.        Attachment E - Attachment E refers to the OUC Authorization Form, but the Authorization Form is entitled Attachment F. Can you please confirm that the Authorization Form, entitled Attachment F, is the document referenced in Attachment E.
12.        Attachment D - Can you please confirm that Attachment D – Technical Risk Questionnaire is the Questionnaire referenced in Attachment C – question #1.4.8 as this references the Technical Risk Questionnaire is Attachment E.
13.        Attachment B - For additional services that we offer that are not included in Attachment B – Pricing Schedule, can we submit an alternate pricing schedule to include such services?
