Solicitation Summary

The Maryland Department of Health (MDH or the Department), Center for Chronic Disease Prevention and Control (CCDPC), is issuing this Request for Applications (RFA) to expand access to diabetes self-management programs in clinical settings. This project will support the broader goal of developing a sustainable network of diabetes self-management support and education (DSMES) sites across the state with the capacity to offer services for education, support, screening, and team-based care that will address the needs of priority populations, including Black men and persons with low income.
