Oral Surgery Services

1)The CONTRACTOR must be available two (2) to four (4) days per month (census dependent), Monday through Friday, (excluding Holidays) between the hours of 8:00 AM and 3:30PM.

2) The CONTRACTOR will provide dental/oral surgery services, at their place of business, for an estimated maximum of 6 patients per month. This amount could be more or less depending on needs of the facility. The schedule will be coordinated through the Capital District Psychiatric Center (CDPC), which allows for direct consultation with the facility staff.

3) The CONTRACTOR is responsible for Extractions, Biopsies, Facial Bone fractures and Oral Maxilla Facial procedures.

4) Confidentiality, as well as, HIPAA guidelines regarding Personal Health Information (PHI) shall be adhered to and is detailed in the attached Appendix F.
