John Hopkins SW Adjusted Clinical Groups (ACG) licenses in support of operational analytics and measures, as well as research and clinical registries needs. The ACG system’s primary design feature is that clustering disease morbidity is a better predictor of resource use than tracking the presence of specific diseases or disease hierarchies. The multi-morbidity framework presents statistically valid and clinically logical information that can then be used for a variety of applications. One of the ACG system’s primary use cases is patient case management, allowing users to identify patients for preventive care, wellness and education services as well as identify patients at high risk for hospitalization, readmission and high pharmacy use. Specifically at DHA, the ACG provides the ability to establish risk scores on individuals in user-defined populations for the purpose of deriving risk and cost information from enterprise data sources, as well as to provide support to empanelment decisions.
