Description
This Sources Sought Notice is for market research and to determine the procurement method for quarterly preventative maintenance services on material handling equipment at the Tucson, AZ, CMOP. Based on the responses to this notice, this requirement may be set aside for small businesses or procured through full and open competition.
This notice is not a solicitation for proposals. Responses are for information purposes only. This notice does not authorize contract negotiations or guarantee funding. Requests for a solicitation will not receive a response, and feedback or evaluations will not be provided to companies regarding their submissions.

Statement Of Work Material Handling Equipment Preventative Maintenance

SUMMARY:
The VA Consolidated Mail Outpatient Pharmacy (CMOP) in Tucson, AZ, operates a high-volume, direct-to-patient prescription medication delivery service for Veterans throughout the United States, utilizing integrated, automated dispensing systems. The Tucson SW CMOP requires a quarterly preventive maintenance plan to keep its Material Handling Equipment (MHE) operational for loading and unloading supplies.
This will be a rm- xed price contract with a duration of one base year and four one-year option periods, which will be exercised at the Government s discretion.

Tucson CMOP, 3675 E. Britannia Drive, Tucson, AZ 85706
OBJECTIVE:
Provide quarterly preventative maintenance on Toyota forklifts and pallet jacks.
SCOPE OF WORK:
The contractor shall provide quarterly services and an annual inspection of all equipment as outlined below.

GENERAL REQUIREMENTS:
All work performed will conform to the Original Equipment Manufacturer (OEM) recommended maintenance speci cations.
All maintenance and repairs must be performed by an OEM service provider.
All parts must be new and conform to OEM speci cations. Rebuilt or remanufactured parts will not be accepted.
Ensure that all equipment maintains its OEM warranties.
The contractor shall provide all materials, supplies, labor, and, if required, an operational substitute piece of MHE
equivalent to the current inventory if repairs cannot be completed within 48 hours.
The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting O cer.
The contractor shall interrupt their work at any time to avoid disrupting facility operations.
The contractor shall have a quality control program to ensure contract requirements are met.

ONSITE REQUIREMENTS:
The contractor must schedule all work with the Contracting O cer Representative (COR).
Before any work is performed, contract personnel must present a government-issued photo ID and sign in to receive
a contractor identi cation badge that must be worn in a visible location while on the premises.
After completing the work, contract personnel shall sign out, return the badge, and submit a report detailing the
work performed, including the make, model, and serial numbers of the equipment that was serviced.

OPERATING HOURS:
Regular business hours are 7:00 AM to 4:30 PM, Monday through Friday. The contractor shall submit a preventative maintenance and annual inspection schedule to the COR prior to the start of the contract.
EMERGENCY REPAIRS:
For emergency repairs, the contractor must dispatch a technician within two (2) hours of being noti ed of equipment failure, Monday through Friday, from 6:00 AM to 4:30 PM. During such repairs, the technician may identify issues that require o -site repairs or the ordering of parts. The technician will then provide a detailed repair estimate, including all associated costs (e.g., parts and labor), for the approval of the Facility Operations Manager.
STANDARDS OF CONDUCT AND SAFETY:
Contract personnel must conduct themselves professionally, avoiding any behaviors that interfere with CMOP operations. Smoking, vaping, possession/use of illegal substances (including medical marijuana), and weapons are prohibited on Government property. Compliance with OSHA and other applicable laws and regulations is exclusively the contractor's obligation. Noncompliance with these standards may be considered grounds for contract termination.
QUALIFICATIONS:
Contractor personnel must be fully quali ed and possess the required equipment, license, and permits to perform this service.
All o erors must be registered in SAM and, if applicable, the VIP databases for SDVOSB or VOSB status.
