Consulting Services for Managed Care Organizations

Project Description: Opportunity Home San Antonio is seeking proposals from qualified and experienced professional consulting firms to assist the organization in developing and implementing a Social Determinants of Health Program initiative. This initiative will create strategies for connecting and partnering with Managed Care Organizations and other potential partners (“Services”). The Consultant will play a crucial role in positioning these resident services as a high-value, cost-effective solution for enhancing member health outcomes, reducing healthcare costs, and maintaining the quality of care. These Services are designed to secure funding for resident programs in Opportunity Home SA’s public housing and affordable housing communities. The selected Consultant(s) will support and facilitate comprehensive outreach efforts with funding agencies for this purpose.
