Subject: Request for Quotation number RFQ 19KU2026Q0012 - RESTROOMS & LACTATION ROOM UPGRADE PROJECT

Enclosed is a Request for Quotations (RFQ) for the Chancery Annex Restroom, Lactation RM & Locker RMs Renovation. If you would like to submit a quotation, follow all solicitation instructions, and complete the required portions of the attached document.

A site visit has been scheduled for August 06, 2026, at 09:30 AM Kuwait local time. For more information, please refer to page 33. Interested contractors are strongly encouraged to attend the scheduled site visit to inspect the premises prior to finalizing their proposals, including the Bill of Materials (BOM) and Bill of Quantities (BOQ). It is the contractor’s sole responsibility to thoroughly assess the site during this visit. Any inaccuracies, omissions, or cost discrepancies in the submitted proposal resulting from failure to inspect the site shall be the contractor’s full responsibility.

The U.S. Government intends to award a contract/purchase order to the responsible company submitting a technically acceptable offer at the lowest price. We intend to award a contract based on initial quotations, without holding discussions, although we may hold discussions with companies in the competitive range if there is a need to do so.

Note: Prospective offerors are strongly encouraged, though not required, to register in the System for Award Management (SAM) at https://www.sam.gov.

• Offerors with an active SAM registration need only provide their Unique Entity Identifier (UEI) number and are not required to complete the information 52.204-90 since relevant information is found in their SAM registration.


• Offerors without an active SAM registration must complete FAR 52.204-90, Offeror Identification, in Section 5, Representations and Certifications. This information must be updated annually if option years are included in the solicitation or award.

Offerors are strongly encouraged to maintain an active SAM account

For a quotation to be considered, you must also complete and submit the following:

1. SF-1442 (14 through 20c)


2. Section A, Price


3. Section L, Representations, certifications, and other statements of offerors or quoters


4. Information and documentation required by Section J, Attachment A, and all other applicable provisions of this solicitation.


5. Proof of SAM Registration / Unique Entity ID, or if not SAM registered, complete FAR 52.204-90, Offeror Identification,

Failure to follow these instructions will render the offer non-compliant and ineligible for further consideration
