City of Columbus Division of Facilities Management Request for Quotation (RFQ) Police Property Room Roof Repair 724 East Woodrow Ave. 1. Repair all visible damage to Coated roof system (Cracks, split, holes) 2. Repair any flashing or coating edge issues Each repair should be cleaned and repaired to comply with standard roofing practices. Contractor will need to clear roof of debris before starting repairs Walk through will be 07/16 at 9 am Miscellaneous 1. Contractor shall provide all necessary labor, material, tools, and equipment to complete the scope of work for this bid. 2. Contractor shall be held responsible for any damage or abuse caused by the negligence of his/her employees.. Contractor is responsible for proper use of the building and equipment. 3. All measurements and quantities are the responsibility of the contractor. Any/all permits if necessary are to be obtained by contractor prior to starting any work. 4. The contractor is responsible for following all safety requirements of the City of Columbus Construction and Materials Specifications. 5. Contractor will give a one year workmanship Guarantee on all work. 6. Contractor is responsible to keep area(s) clean of debris and are responsible for removal of all debris and excess materials on a daily basis. The use of City of Columbus dumpsters will not be permitted. 7. Contractor's employees must clear a local/state/federal background check covering 10 year period or beginning at 19 years of age . Background check provided by the City of Columbus - to be turned over to CA after contract has been signed at contract start Questions should be sent to Brad Bayliff at babavliff@columbus.gov.
