services for the Grande Prairie, Northern Addictions Centre, Courtyard Paving Stones Replacement (“Project”).
Replace Courtyard paving stone subsurface. Provide temporary hoarding that is sealed, dust proof, and ventilated to the exterior, for execution of the work. Remove existing paving stones. Excavate and construct new structural slab subsurface. Install new resilient flooring, base, and accessories. All equipment used inside the building, including equipment to excavate and to drill the
piles, must be electric. No fuel fired equipment will be permitted inside the building.
Proposals should be submitted as three separate files in the manner set out below:
a) Package #1: Technical Submission:
i. This package should include: (1) the information and documentation requested in the Technical Proposal Submission Form; and (2) the signed declaration in Schedule B (Declaration).
ii. This package should have the file name “Technical Submission”.
iii. This package should not include any financial information.
The Technical Proposal should be submitted in sequential order with the following headers:
1. Corporate Profile
2. Proposed Contractor Team including a list of Key Personnel
3. Project Experience
4. Project Comprehension
5. Proposed Work Methodologies b) Package #2: Financial Submission:
i. This package should include the signed Financial Proposal Submission Form.
ii. This package should have the file name “Financial Submission”.
c) Package #3: Surety Submission:
iii. This package should include the bid bond set out in Section 2.10 (Bid Bond)
issued by the Proponent’s surety company.
iv. Clearly indicate in the file name “Surety Submission”.
