I. PURPOSE
The City of Unalaska (hereafter “City”) is seeking to retain a qualified professional safety consultant with
qualifications and expertise to provide safety program consulting services to the City.
The City requests the services of a safety consultant to help develop and implement all required safety
programs to ensure the City remains in compliance with OSHA and AKOSH standards and protect the
health and well-being of its employees. The consultant will work closely with risk management,
department management and City administration to provide services as outlined in the scope of services.
The City will enter into a Professional Services Agreement (see Attachment A, Form of Agreement and
Attachment B, Insurance Requirements) with one individual or firm to perform an agreed-upon specific
scope of services and fees that best meet the safety program needs of the City.
II. BACKGROUND
Unalaska is the 12th largest incorporated city in Alaska and is 800 air miles southwest of Anchorage in the
heart of the healthy and robust North Pacific/Bering Sea fisheries. Our community is a vibrant mix of
industry and history connected by 44 miles of roads linking our port, harbors and private docks with local
businesses and our thriving residential community of 4,120. We work hard to provide steady support to
one of the busiest and most prosperous stretches of coastline in Alaska.
Travel is primarily via air, so this geographical remoteness creates many unique challenges that require
creative solutions. Three years ago, the arrival of high speed internet service to our island community was
a game changer, and we are building upon the opportunities that are now possible.
The City has a Mayor-Strong City Manager form of government, which consists of a Mayor and six City
Council Members who are elected, and hire the City Manager. The city provides a wide range of services,
which include:
City Hall: Finance, Information Services, City Manager’s Office, Planning, City Clerk, and
Administration (Human Resources and Risk Management)
Parks, Culture and Recreation: Public Library, Aquatics Center, Community Center, Parks and
Outdoor Facilities
Police: Corrections, Communications, Animal Control and DMV
Fire and Emergency Medical Services
Public Works: Engineering and Administration, Supply, Roads, Vehicle Maintenance, Facilities
Maintenance
Public Utilities: Electric Production, Electric Distribution, Water, Wastewater, Solid Waste
Ports and Harbors: Port and Harbor Facilities, Airport Terminal
Because the City of Unalaska is a small local government in a remote location, proposals should utilize
creative, cost-effective solutions. While price is a significant factor, a quality safety and training program
are the goal and will form the basis of the contract award decision.

III. SCOPE OF SERVICES
The City is accepting proposals from qualified consultants to provide safety program consulting, training
and assistance. The consultant shall provide qualified safety professional(s) who can provide on-site
services periodically at the City, and remote support as needed. The scope of services shall include but
are not limited to the items noted below.
If the consultant feels that additional tasks are warranted, they should be clearly identified in the
consultant’s proposal.
Task #1: Assess the City’s current safety programs
1. Assess the City’s safety programs and identify deficiencies.
Task #2: Assist city staff in correcting deficiencies and developing a
comprehensive safety program that includes the following OSHA required
elements
1. Job Hazard Assessments: Assist with completion of written job hazard assessments to
determine/clarify/update the required safety training, personal protective equipment, shoes,
gear, etc. for each city position.
2. Written Plans: Written safety plans to ensure compliance with federal and state standards,
regulations, laws and city ordinance and rules, as well as assist in the development of any new
plans as needed. This includes, but is not limited to:
a. Accident Investigation
b. Aerosol Transmissible Diseases
c. Asbestos and Lead
d. Bloodborne Pathogen and Exposure Control Plan
e. Chemical Hygiene Plan
f. City of Unalaska Master Safety Plan
g. Codes of Safe Practices
h. Confined Space
i. Defensive Driving
j. Department/Division Safety Plans
k. Electrical Safety
l. Emergency Action Plan

3. Safety Training and Program Management Online Platform: Assist in finding the best, most
comprehensive and economical online training platform that contains:
a. Quality safety training that meets city departments’ varying safety training needs
b. Written program guidelines templates
c. JHA template
d. SOP template
e. Bilingual options (Tagalog, Samoan, Spanish, Vietnamese, etc.)
Task #3: Develop a Facility Safety Inspection and Compliance Process
1. Provide staff training to identify hazards (physical, equipment and behavioral).
2. Establish inspection procedures, and best practices to evaluate facilities to assess risk and
compliance with federal, state, and local regulations.
3. Develop hazard reporting forms and steps to correct the hazard.
Task #4 Leadership Training for Supervisors, Managers and Safety Committee
1. Safety Committee: Assist in developing safety leadership skills and how to create a safety culture.
Provide guidance for safety program development and implementation. Recommend safety-
related committee projects.
2. Managers/Supervisors: Assist in developing leadership skills and how to create a safety culture.
Provide guidance for safety program development and implementation. Recommend safety
incentives and how to encourage safe behavior in the work environment
Task #5 Provide Ongoing Support
1. Annually participate in updating the master safety plan.
2. While working with the city, provide a monthly progress report to Risk Management.
3. Proposers are encouraged to recommend any additional options, recommended services, or
tasks.

IV. PROPOSAL REQUIREMENTS
Proposal shall contain the following:
1. Cover Letter. The Cover Letter shall be addressed to Debra Hanson Zueger, Risk Coordinator, and
at minimum, must contain the following:
a. Identification of firm, including name, address, and telephone number of contact person
during period of proposed evaluation;
b. Statement to the effect that the proposal shall remain valid for a period of not less than
120 calendar days from the date of submittal; and
c. Signature of a person authorized to bind the firm to the terms of the proposal
2. Executive Summary. In a brief narrative, summarize the proposed solution by setting forth the
overall approach and plans to meet the requirements of the tasks. The intent of this narrative is
to convey to the City that the firm understands the objective of the requested service, the nature
of the work, and the level of effort necessary to successfully provide the defined services.
3. Scope of Work/Methodology. For tasks 1-4 above, provide a detailed description of the approach
and methodology to be used to accomplish the tasks. The methodology should include:
a. A detailed description of the approach to accomplishing the task.
b. the specific tasks and roles required by City staff.
4. Rates. Provide the consultant’s rate schedule and written statement that said rate schedule is part
of the consultant’s quote for use in invoicing for progress payments and for extra work incurred
that is not of this RFP.
5. Schedule. A tentative schedule of the amount of time for the project/tasks. The exact dates will
be determined jointly by the consultant and the city.
6. Required Statements
a. A written statement by the consultant that all federal laws and regulations shall be
adhered to notwithstanding any state or local laws and regulations. In case of conflict
between federal, state, or local laws or regulations, the strictest shall be adhered to.
b. A written statement that consultant will not discriminate against any employee or
applicant for employment because race, color, religion, sex, or national origin.
c. That the consultant will provide all necessary personnel, and list what materials,
equipment and staff shall be provided by the consultant and what materials, equipment
and staff shall be supplied by the City to perform the services described.
7. References
a. Provide at least three (3) references using the same service being proposed and indicate
the scope of work, date, name, email address, and telephone number of the client
contact.
