Description:	The North Carolina Department of Public Safety, Division of Emergency Management (NCEM) is soliciting proposals to acquire the services of a qualified Vendor(s) to provide Emergency Medical Services (EMS) in North Carolina during a disaster response or medical surge event that overwhelms available resources. These services shall provide assistance as needed in all areas of emergency medical services such as; ground basic, advanced life support and specialty care EMS services, paratransit, rotor wing, and fixed wing, and EMS and Nursing Personnel which are licensed by applicable State authorities to provide those services, and to include but not limited to Liaison Officer, Logistics Officer, and EMS Crew Configuration. The EMS services may be required to perform their assignments at the State Emergency Operations Center (SEOC), in State-Coordinated response efforts, virtual or in other areas as assigned in North Carolina. This solicitation is for qualified vendors located anywhere in the United States, to include North Carolina. No minimums or maximums are guaranteed.

In compliance with this Invitation for Bid (IFB), and subject to all the conditions herein, the undersigned Vendor offers and agrees to furnish and

deliver any or all items upon which prices are bid, at the prices set opposite each item within the time specified herein.

By executing this bid, the undersigned Vendor understands that false certification is a Class I felony and certifies that:

 this bid is submitted competitively and without collusion (G.S. 143-54),

 none of its officers, directors, or owners of an unincorporated business entity has been convicted of any violations of Chapter 78A of

the General Statutes, the Securities Act of 1933, or the Securities Exchange Act of 1934 (G.S. 143-59.2), and

 it is not an ineligible Vendor as set forth in G.S. 143-59.1.
