Title: “CITYWIDE EAP SERVICES PROPOSAL #SV-25-P-050/TB”
The City of Titusville is requesting proposals from qualified Employee Assistance Program (EAP) providers to deliver professional, confidential EAP services to approximately 600 employees, including law enforcement officers, firefighters, professional, and blue-collar (public works, water/sewer, etc) municipal personnel, and their eligible dependents (spouse, children under 26, incapacitated dependents, etc.). The mission is to promote a healthy and safe work environment by providing resources to address issues impacting job performance or work behavior, enhancing emotional health and wellness. The purpose of this RFP is to establish a multi-year contract with one (1) qualified firm to promote mental, emotional, and overall well-being through comprehensive counseling, crisis intervention, educational programs, and referral services, with a seamless transition of care to the City’s current medical plan provider for treatment required beyond the EAP plan.

Incomplete proposals or those from vendors on the convicted vendor list (Section 287.133, F.S.) will be disqualified

The proposer must submit one (1) hardcopy and one (1) digital copy on USB flash drive or CD/DVD, in a searchable Adobe PDF format (.pdf) or .Zip file format unless otherwise stated within the Solicitation Packet. All submittal materials must be in a sealed envelope and/or box and must be received no later than the due date listed above.
