Title: “CITYWIDE EAP SERVICES PROPOSAL #SV-25-P-050/TB”

The City of Titusville is requesting proposals from qualified Employee Assistance Program (EAP) providers to deliver professional, confidential EAP services to approximately 600 employees, including law enforcement officers, firefighters, professional, and blue-collar (public works, water/sewer, etc) municipal personnel, and their eligible dependents (spouse, children under 26, incapacitated dependents, etc.). The mission is to promote a healthy and safe work environment by providing resources to address issues impacting job performance or work behavior, enhancing emotional health and wellness. The purpose of this RFP is to establish a multi-year contract with one (1) qualified firm to promote mental, emotional, and overall well-being through comprehensive counseling, crisis intervention, educational programs, and referral services, with a seamless transition of care to the City’s current medical plan provider for treatment required beyond the EAP plan.

Proposals must remain valid for 120 days. Incomplete proposals or those from vendors on the convicted vendor list (Section 287.133, F.S.) will be disqualified .

The proposer must submit one (1) hardcopy and one (1) digital copy on USB flash drive or CD/DVD, in a searchable Adobe PDF format (.pdf) or .Zip file format unless otherwise stated within the Solicitation Packet. All submittal materials must be in a sealed envelope and/or box and must be received no later than the due date listed above.

Qualifications:

Proposers must be registered in Florida (www.sunbiz.org), to conduct business in the State of Florida, hold all appropriate Professional credentials and licenses, and have provided EAP services at least three (3) other accounts of similar size and demographics serving municipalities and first responders within the past 5 years. In addition, must have demonstrated capacity to manage confidential health/behavioral services.

Special Requirements:

Counselor’s Minimum Qualifications
Staff delivering counseling services must have at least a master’s degree from an accredited college or university in a behavioral science, with a minimum of two years of practical related work experience and must be appropriately licensed in Florida (Psychology, Clinical Social Work, Mental Health Counseling, Marriage & Family Therapy, or Medicine/Osteopathic Medicine).

Exception to the master’s degree requirement may be made for licensed, accredited or certified staff in fields requiring specialized knowledge, training and expertise obtained through professional training programs that do not award academic degrees. Exceptions (e.g., for Substance Abuse Professionals) must be justified in the proposal and approved by the City.
