Description:

The Department of Human Services (“DHS”), Wernersville State Hospital (“WeSH”) is seeking a contractor to provide Basic Life Support (“BLS”) non-emergent ambulance services for consumers at WeSH, 160 Main Street, Wernersville, PA, 19565.

The quantities are estimated only and may increase or decrease dependent upon the needs of the department.

Bids must be submitted electronically. Late bids will be disqualified. Contractors must enter firm prices for all contracted years and provide appropriate responses to all “Mandatory Attributes” contained in the solicitation. Failure to submit bid as described may result in yours being rejected as a non-responsive bidder.

Award will be made to the lowest responsive and responsible bidder.

Vendor is to complete the following attachments and submit with bid response:

Lobbying Certification
Prior to commencement of the work under the awarded Standalone Purchase Order:
1) the selected Contractor shall provide WeSH with current certificates of insurance; and
2) qualifications/eligibility/certification of current emergency medical services personnel and license of ambulance services shall be submitted and remain on file at WeSH for the duration of the contract.
