Project description
The project is an initiative of Health Quality Alberta to obtain feedback from residents and families of residents of Type A and Type B Continuing Care Homes (CCH), about the quality of care and services residents receive. This feedback is used to describe the current state of CCH from the perspective of residents and their families and to provide CCH and other health system partners with information that can be used for ongoing monitoring and improvement.
This feedback is captured through Health Quality Alberta’s Continuing Care Homes Resident and Family Experience Surveys. Trained survey interviewers, hired and managed by the vendor, will conduct face-to-face interviews with residents in CCH across Alberta. A self-administered paper survey will also be distributed by the survey interviewers for residents who are able to complete one. The vendor will also conduct a web survey (self-administered) of families of residents living in CCH in Alberta. This survey protocol includes a 3-stage mail survey for family members who do not have an email address.
Health Quality Alberta is seeking a vendor to oversee the data collection of the resident and family experience surveys.

This RFP is split into two parts:
Part I – CCH Family Experience Survey
Part II – CCH Resident Experience Survey

Project Deliverables
The objective of the project is to conduct a survey of CCH residents and resident family members (through in-person, direct mail, web-based, and telephone methods). This work will include the data capture of all elements of this survey and generation of data files according to the project specifications outlined in the methodology sections in Part I and Part II of this RFP. Other aspects of the broader study such as related analysis and reporting are not part of this RFP.

Proposal Format
Vendor understanding of the requirements for Health Quality Alberta’s Continuing Care Homes Resident and Family Experience Surveys project.
Project approach and methodology including a detailed approach to be used including processes, methods, workflow, tasks, data collection methodology, and process to protect data.
Engagement team to include a description of the personnel assigned, including resumes, availability, qualifications, responsibilities, and an estimate of hours by activity for each individual. This includes demonstrated capacity for data capture of survey items, especially the family experience survey’s open-ended item, question 64. Include the structure of the team for the project.
Detailed fee proposal

Deadline for Submission
All submissions shall be firm quotations and may not be withdrawn for a period of sixty (60) days following the last day to accept quotations. Quotations and documents submitted by the vendor will remain the property of Health Quality Alberta and will not be returned.

Acceptance/Rejection of Responses:
Health Quality Alberta reserves the right to cancel this RFP at any time and to reissue it for any reason without incurring liability and with no vendor having any claim against Health Quality Alberta as a consequence.
Health Quality Alberta reserves the right to reject any or all proposals; the lowest fee proposal will not necessarily be awarded a contract.
All vendors submitting proposals will be advised of the contract award.
Any and all addenda to this RFP will be issued in writing and sent to all firms having received documents from Health Quality Alberta prior to the closing deadline.

Closing of Proposal:
Firms may not submit new price proposals after the specified deadline.
Amendments to submitted proposals must be received in writing prior to the deadline date.
All addenda issued during the time of the Request, and in closing, the addenda will become part of the contract along with the response to the proposal.

Confidentiality and use of Information
All proposals received are confidential and shall be treated as such. All documents submitted to Health Quality Alberta are subject to the protection and disclosure provisions of the Access to Information Act (ATIA), the Protection of Privacy Act (POPA) and the Health Information Act (HIA). While these Acts allow a person a right of access to records in Health Quality Alberta’s custody or control, it also prohibits Health Quality Alberta from disclosing personal or business information where disclosure would be harmful to business interests or would be an unreasonable invasion of personal privacy. Applicants are encouraged to identify what portions of their submissions are confidential and what harm could reasonably be expected from its disclosure.

The selected vendor is required to manage identifiable personal and health information in compliance with the HIA, ATIA, and the POPA, as an agent under contract with Health Quality Alberta. Specific confidentiality and privacy requirements are included in Health Quality Alberta’s contract. These requirements will apply to all sub-contractors, and all sub-contractors engaged by the vendor must be identified clearly to Health Quality Alberta.

Professional Liability Insurance
7.1        The Vendor shall, at its own expense and without limiting its liabilities herein, insure its operations under a contract of General Liability Insurance, in accordance with the Alberta Insurance Act, in an amount not less than $2,000,000.00 inclusive per occurrence, insuring against bodily injury, personal injury and property damage including loss of use thereof and shall provide evidence of such insurance to Health Quality Alberta upon request.

Throughout the term of this Agreement the Vendor shall maintain professional liability insurance in an amount of not less than $2,000,000.00 and shall provide evidence of such insurance to Health Quality Alberta upon request.

The Vendor shall maintain automobile liability insurance on all vehicles owned, operated or licensed in the name of the Vendor in an amount not less than $2,000,000.00 and the Vendor shall provide evidence of such insurance to Health Quality Alberta upon request.
