Solicitation Summary
Expand the knowledge and capacity of adult primary care and emergency department providers to manage Sickle Cell Disease (SCD) by developing or enhancing a “Hub and Spoke” model of care in underserved areas, thereby reducing acute care usage as required by Md. Code Ann., Health-General §18-507.a.7. The Hub and Spoke model of sickle cell care gives existing centers the capacity to reach beyond their walls and support centers in communities where sickle cell centered care is not available.
