Summary

DEPARTMENT OF REHABILITATION AND CORRECTIONS REQUEST PRODUCT AND INSTALLATION QUOTE

On behalf of the Department of Rehabilitation and Corrections (DRC), please provide a quote for installation services of (134) Herman Miller AO2 workstations (or approved equal), (18) manager soft-office cubicles , and (1) reception configuration with two cubicles l ocated at 4545 Fisher Road, Suite D, Columbus, Ohio 43228. The work will be done during normal business hours. All services are to be completed w ithin three weeks of award .

GENERAL INFORMATION
• Vendor will obtain and abide by all rules and regulations from the state entity and the building property manager.
• Vendor will attend project meetings, including but not limited to pre-installation, pre-construction, progress meetings, and punch list walk-through.
• Vendor will be responsible for keeping the site orderly, clean, and safe at all times.
• Vendor will be responsible for wiping down workstations after installation and clearing the space of trash and all packing materials

SCOPE OF WORK
• Design Services
Prepare CAD drawing of modular furniture, layout, and provide a quote for the required product.
• New Product
All modular workstation pieces and parts are needed to complete the layout.
• Installation Services
Install (134) Herman Miller AO2 workstations (or approved equal), (18) manager soft-office cubicles, and (1) reception configuration with two cubicles

PROVIDE ONE QUOTE for Design, Product, and Installation Services. Provide the cost for each service as a separate line item. Each line item on the quote should be detailed to include the quantity of hours and cost per hour. (The quote should be detailed to include each product with description and product number. Quote should indicate with separate columns the item List Price, Discount Percent, and Purchase Price.)
SERVICES TO BE PROVIDED
DELIVERABLES:
1. PDF of installation plan to review and approval for agreement with the approved layout.
2. Detailed parts list for all components needed to complete the installation. including all finish information.
3. CAD file and PDF of as-built plan.
4. Provide the workstation's electric wiring diagram cut sheet for the electrician's reference.

MANDATORY SITE VISIT

Participation in a mandatory site visit is required for bid submission.
