SUBMITTAL - Scope of Work, Response

Technical Qualifications

Ensure all welding personnel maintain current certifications appropriate for stainless-steel work (e.g., AWS D1.6 or equivalent). Certifications shall be provided with the Offer or within three (3) business days upon request by the City.

Attach Certifications

Possess a minimum of five (5) years of documented experience performing stainlesssteel welding and plasma cutting work of similar scope and complexity. This can be provided in a resume of work with the response offer or within three (3) business days upon request by the City. The list shall include the facility name, location, contact person, and phone number.

Provide a list of three (3) similar cutting and welding projects completed within the past five (5) years, with the response offer or within three (3) business days upon request by the City. The list shall include the facility name, location, contact person, and phone number

Demonstrate successful completion of at least one (1) project within the past five (5) years involving confined-space work at a water or wastewater treatment facility, including welding, cutting, or other hot work performed in a permit-required confined space. This information shall be provided with the Offer or within three (3) business days upon request by the City and must include the facility name, location, contact person, and phone number.

Regulatory and Administrative Qualifications

Provide documentation that all personnel designated as Authorized Entrants, Attendants, and Entry Supervisors have completed confinedspace entry training from an accredited provider within the past twelve (12) months, with the response offer or within three (3) business days upon request by the City.

Water Utility and Facility Specific Qualifications

Identify a designated Person qualified responsible for confinedspace hazard evaluations, permit authorization, and oversight of confinedspace activities throughout the duration of the Work.
