SUBMITTAL - Scope of Work, Response

The Offeror shall provide proof of the following minimum qualifications. Offeror shall attach written evidence to THIS document; documented proof of each applicable minimum qualification listed below from Section 4.0 of the Scope of Work. Each attachment to this document shall be signed by Offeror or an authorized representative. THIS completed document shall be labelled "SUBMITTAL - SOW RESPONSE" and be uploaded alongside other required SUBMITTALS including SUBMITTAL - PRICES AND PRICING.

Contractor's Minimum Qualifications

Contractor shall:

Have a minimum of three (3) years' experience within the past (6) six years in commercial window cleaning using powered platforms for exterior building maintenance, cleaning windows of high-rise buildings, and have experience in the use of equipment including, but not limited to: powered platforms, staging equipment, Bosun's chairs, and lifts required to access the high and low level windows of such buildings. Aviation requires references for the above and at its sole discretion may validate such references. Find and complete the References template on page 2 of this document.

Possess at least five (5) years' experience managing subcontractors if it uses subcontractors.

For Offerors that plan to use subcontractors, please provide Subcontractor name and contact information (phone and email) and the years for which work was completed for each Subcontractor. Sign and attach to this document.

Obtain all permits and licenses that are necessary and that are required by law and regulation, to perform the work prior to the start date.

Please attach all current applicable Offeror permits and/or licenses or provide list of current permits and/or licenses that the Offeror must possess prior to the start of services. Sign and attach to this document.

Assign staff, including supervision staff, to the Contract that complies with the Aviation Airport Security Plan and are able to obtain Airport Security Badges as required by Aviation. To obtain an Airport Security Badge, all Contractor's employees and subcontractor's employees that provide services, including periodic supervision and site visits, must be able to successfully complete the Criminal History Records Check and Security Threat Assessment process, and badge and security training.

OFFEROR REFERENCES: The City at its discretion may check references to determine the Offeror's experience and ability to provide the products and/or services described in this Solicitation. The Offeror shall furnish at least 3 complete and verifiable references. References shall consist of customers to whom the offeror has provided the same or similar services within the last 5 years. References shall indicate a record of positive past performance.
