Ductless Heat Pump Installation
The Quinault Housing Authority seeks an HVAC contractor to install a heating system in two
homes.
Electrical work must be performed by a licensed electrician.
Site visits will be required to determine the best way to heat/cool the area needed and to view
actual site conditions.
708 Cuitan – Install ductless heat pump.
1414 Aalis – Install ductless heat pump and replace “Octopus” ventilation assembly.
I. Indian Preference (20 pts)
The Quinault Indian Nation has enacted a TERO Ordinance governing Indian Preference,
and the Quinault Housing Authority has adopted an Indian Preference Policy, set forth in
section 3000.10 of the QHA Procurement Policy. To the greatest extent feasible,
preference shall be given to Indian organizations and Indian-Owned economic enterprises
in the award of all contracts and subcontracts.
A. An Indian, Indian –Owned economic enterprise or Indian organization is eligible for
Indian Preference (15 points)
B. Proposals shall include a statement describing how you will provide for Indian
preference in subcontracting, training and employment in this contract, including
the number or percentage of Indians to be employed and trained, in accordance
with Section 3000.10 of the QHA Procurement Policy. All proposals must include
this statement. (5 points)
II. Price (60 pts)
Provide total cost to complete the project.
III. Experience (20 pts)
Include a statement of experience performing similar work, include any experience working for
housing authorities or Native American organizations.
If you do not have experience performing this type of work for QHA please include references
of recent similar jobs that you have completed with a similar scope of work.
Other provisions
The contractor selected must have all required insurance, including worker’s
compensation, general liability insurance of not less than $500,000.00 per occurrence
and $100,000.00 aggregate.
Contractor selected will be required to complete a TERO compliance plan and pay the
1.75% TERO fee; per QTC 97.03.040 (d) the TERO fee cannot be included in the bid price.
The contractor will be required to follow all TERO regulations; including hiring from the
TERO hall.
Project is subject to Tribally determined wage rate.
Contractor will be required to possess a QIN business license.
All proposals must include the name of all owners of the company.
All proposals must include a completed Form HUD 5369C
The contractor selected must submit a W-9 if QHA has not worked with you before.
Unless contractor is a sole proprietorship Workman’s Compensation coverage will be
required.
As a housing authority in Washington State QHA is exempt from all taxes.
Permit to be obtained by the contractor.
To the greatest extent practicable preference shall be given for the use of materials
produced in the United States.
If the total contract price is $25,000 or higher, a 5% bid bond must accompany the bid.
The selected contractor will be required to submit a fully executed Performance Bond for
100% of the contract price within 10 days of award notice
The selected contractor will be required to submit a fully executed Payment Bond for
100% of the contract price within 10 days of award notice
