Addendum governs. Acknowledge receipt of the Addendum by inserting its number in Document 00310 -
Form of Proposal. FAILURE TO DO SO MAY SUBJECT BIDDER TO
DISQUALIFICATION.

[1. Revisions were made to Document 00405 Schedule of Unit Price Work. Items removed associated with
8-inch main were removed. Use attached form.
CHANGES TO DRAWINGS
[1. Revisions were made to Sheet C-3.1 Water and Sanitary Sewer Plan. Project will only include 4-inch
sewer service line. 8-inch extension has been constructed by others. See attached plan sheet.
