Description:
The California Department of Public Health (CDPH) is soliciting responses from qualified vendors to
provide services that will implement an evidence-based approach to help decrease the risk for type
2 diabetes, hypertension (HTN), and high blood pressure (HBC) among priority populations and
enhance quality of care and early detection of diabetes, HTN, HBC, and their complications by
providing training and technical assistance including webinars and virtual meetings to the Type 2 Diabetes Program (T2DP) and HeartBeatCA partners and to pharmacy networks in California. The contract term will be from April 15th, 2025, to June 29th, 2028, with an option to amend for an
additional year.
