SIGA Flu and Wellness Clinics

The Saskatchewan Indian Gaming Authority (SIGA) seeks to engage one qualified and experienced vendor (the “Flu and Wellness” Supplier) to provide Flu and Wellness Clinics described below in accordance with the terms and conditions hereinafter. The successful Bidder (the “Flu and Wellness” Supplier) shall provide concurrently the Flu and Wellness services, for SIGA’s Central office and its seven (7) casino sites located in the cities indicated at F.O.B Delivery Points.

The successful bidder (the “Flu and Wellness” Supplier) is to provide the required services between the hours of 9:00AM and 5:00PM, Monday to Friday for all sites. Flu and wellness clinics for each location will be held in one day for each location. May vary based on need. Dates and times will be determined in consultation with SIGA’s Health and Wellness Coordinators and the successful supplier of these services. The successful bidder shall provide all the required services as indicated in the ITT document.

SIGA would be looking to retain the successful Supplier for a period of two (2) years with three (3) annual renewal options for a total maximum term of five (5) years in the form of a Services Agreement.

REQUIREMENTS OF QUALIFYING VENDOR
1. A fully completed bid submission, with all required sections of this document completed and signed where applicable.
2. Evidence demonstrating the vendor’s capacity and capability to perform and deliver the required services (i.e. experience delivering workplace or corporate flu vaccination and wellness clinics).
3. All pricing shall be in Canadian Dollars $CAD before taxes, as applicable.
4. Cholesterol testing: provide total Cholesterol only with no lipid breakdown for HDL, LDL, triglycerides and ratios (test must be non-fasting).
5. Glucose/blood sugar testing - provide Glucose finger stick test, must be non-fasting.
6. The successful bidder must ensure the safe and compliant disposal of used needles and other clinical waste in accordance with Saskatchewan Health regulations. All health and safety requirements and best practice guidelines to be strictly adhered to at all times.
7. The awarded vendor shall provide a valid registered membership number with Workers Compensation Board (“WCB”), Saskatchewan.
8. The successful bidder is required to provide all credentials of practitioners that are authorized to provide these complete set of services (i.e. Appropriate professional licensing and certifications).
9. Should the Bidder be awarded this Tender, the vendor is required to provide a certificate of insurance or letter from an insurance company in good standing confirming the Bidder’s ability to obtain the required insurance coverage.
10. The awarded vendor shall ensure that the insurance coverage is maintained in force for both professional liability and comprehensive general liability insurance, including covering bodily injury, including death, and property damage in an amount of not less than five million dollars ($5,000,000.00) per occurrence.&
