Wisconsin Department of Health Services Chronic Disease Prevention Program (CDPP) is seeking to partner with an organization which holds an umbrella license to all Self-Management Resource Center (SMRC) programs, including the Diabetes Self-Management Program (DSMP) known as Healthy Living with Diabetes or Vivir Saludable con Diabetes, and provides training, technical assistance, and support to current or aspiring SMRC DSMPs.
Scope of work- Determine current landscape of past and present SMRC DSMPs located within CDPP-identified high-need counties and identify gaps in these programs.
- Identify and connect with past SMRC DSMP programs located within CDPP-identified high-need counties to determine capacity to resume the program.
- Provide financial and programmatic support to SMRC DSMPs.
- Work with established SMRC DSMPs to promote workshops, identify and address barriers to attendance, and determine strategies to sustain programs.
