REQUEST FOR PERSONAL SERVICES CONTRACT

The Ohio Department of Commerce (COM) Division of State Fire Marshal (SFM) on behalf of the Ohio Fire Academy (OFA) is requesting a personal services contract in order to contract for trained and certified instructors to assist in delivering course content and materials to fire departments throughout Ohio — both paid and volunteer — in delivering continuing education content on fire fighter mental health and wellbeing and peer support programs for first responders in the state of Ohio.

Department Background
The Ohio Department of Commerce (ODOC), Division of State Fire Marshal’s (SFM) mission is to safeguard the public, its property, and the environment from fire and related risks through education, investigation, and enforcement.
The Ohio Fire Academy, a bureau within the Division of State Fire Marshal, is responsible for providing training to Ohio’s first responders (firefighters, emergency medical services, and law enforcement).
The mission of the Ohio Fire Academy is to provide fire service personnel and other allied professionals with training, education, and accredited courses consistent with nationally recognized standards to improve the safety and proficiency of Ohio’s Fire Service.
It is the bureau’s vision to be recognized as the national premier leader in the development and delivery of fire, EMS, and emergency responder training programs. Project Objective
The Ohio Fire Academy and the Division of State Fire Marshal are responsible for ensuring that training is available to all fire departments and first responders as outlined in our mission and assignment of function in the ORC.
Peer‑based instructors are uniquely positioned to deliver behavioral health training to the fire and EMS community through trusted, peer-based connections with responders across the state. These instructors are firefighters and EMS professionals who understand the culture, operational realities, and occupational stressors of the fire and emergency medical services.
This personal services contract will enable OFA/DSFM to deliver needed training to volunteer and paid first responders in the state of Ohio, where a current training gap exists due to OFA not having trained and certified staff available to deliver this training. Project Budget
Not to exceed $42,000 .
Forty classes total, to be invoiced under the scope of work described.
Agreement Term
Contract effective date – July 1, 2026 through June 30, 2027 .
This contract can be renewed based on agreement of both parties and continued appropriation of funding each fiscal year.
SCOPE OF WORK / DELIVERABLES
Scope of Work Requirements
The OFA will function as the administrative oversight in this contract and ensure instructors have access to all needed ACADIS access, course postings, registrations, and course completion certificates for each class attendee.
Instructors will be responsible for advertising, scheduling, teaching, all course development, and travel costs for its instructors within the prescribed and agreed‑upon cost‑per‑class schedule .
Instructors will send OFA a monthly invoicing statement noting the date, course location, number of attendees, and charge for each class. OFA will ensure all invoices are processed and drawn down against the Project Total.
Instructors will be responsible for delivering the following classes statewide: six courses intended for general Fire/EMS providers and three courses for peer support continuing education in accordance with ORC 2317.023. COURSE DESCRIPTIONS Fire Service Mental Health 101

Process

Itemized Quote. The quote shall be itemized and include the following: estimated shipping, company name, contact information, including name, phone number and email to which COM should remit the purchase order.
Nothing herein guarantees that the quote will be accepted by Commerce.
Form of Payment. Please state in the quote if you accept credit card as form of payment and if there are any additional fees applied to those purchases.  If so, please state the additional fees and/or surcharge in the quote.
Current State Contract or Agreement. If the products and/or services are currently contra cted under a Department of Administrative Services (“DAS”) Contract, State Term Schedule (“STS”), Master Maintenance Agreement (“MMA”) or Master Cloud Services Agreement (“MCSA”), provide the contract number and confirm your quote pricing matches what is listed in the pricing section of the DAS contract.  If the pricing contained in your responsive documents fails to match the DAS contract pricing, your quote may be rejected.
If the product and/or service requested is not quoted from an existing DAS Contract or Agreement, please provide supplier’s Software License Agreement, Maintenance Agreement, and Terms and Conditions documentation when quoting.
If not all products and/or services are referenced on a DAS Contract, please provide two separate quotes: one for products and/or services addressed in the DAS contract and one for the products and/or services not addressed by the DAS contract.
If signature is required for products and/or services not addressed by the DAS contract, please provide the agreement and/or terms and conditions related to the purchase. This includes software license agreements.
