The purpose of this Request for Quote (RFQ) is to establish a contract or issue a purchase order with the responsive and responsible vendor that submits the lowest priced quote for the following equipment: Hamilton T1 Transport Ventilator (Adult, pediatric, and neonatal) With Stand, including all required components, software options, accessories, and delivery. Quantity: 1 Please Note NO SUBSTITUTIONS WILL BE ALLOWED. We need to purchase the Hamilton T1 Transport Ventilator (Adult, pediatric, and neonatal).
Must Include:
* AC power cord
* O₂ Hose
* Stand / Mobile trolley
* User/Operator Manual
* Battery
Product Condition Requirement
The Hamilton T1 Transport Ventilator may be supplied as either:
* New, or
* Refurbished / Recertified Certified ( Preferred )
Snow College prefers a refurbished certified unit, provided that the ventilator is fully patient‑ready, includes all required accessories, has undergone complete functional testing, and carries a valid warranty.
Delivery Deadline:
Snow College requires the Hamilton T1 Transport Ventilator, stand, accessories, and all documentation to be delivered AS SOON AS POSSIBLE (ASAP).
Vendors must provide their earliest achievable delivery date and make all reasonable efforts to expedite fulfillment.
Additional Notes: - Delivery must be FOB Destination to the specified location in Utah. All shipping and handling costs must be included in the quoted price.
Evaluation of Quotes: Each quote received shall be evaluated for responsiveness as outlined in the Utah Procurement Code. This RFQ shall be evaluated by the objective criteria described in this RFQ. Quotes submitted must comply with the specification identified in this RFQ.
Multiple or alternate quotes will not be accepted. If a vendor submits multiple or alternate quotes, Snow College will only accept the bidder's primary bid and will not accept any other bids constituting multiple or alternate bids.
Snow College Purchasing may solicit other quotes via email, phone, or other methods as permitted by the Utah Procurement Code or applicable Administrative Rules.
