Summary
Optometry
The contractor will deliver comprehensive optometry services to patients referred by NOBH. They will prescribe and supply eyeglasses and other hospital-approved optical aids to correct visual impairments. The contractor will perform examinations, diagnose, treat, and manage conditions, injuries, and disorders related to the visual system. Optometry services must be provided by licensed optometrists who are authorized to practice in the state of Ohio and in accordance with applicable federal, state, and local regulations.

Podiatry Services
The contractor will provide specialized medical care in podiatry for patients referred by NOBH. This includes the examination, diagnosis, evaluation, and treatment of both acute and chronic foot conditions using palliative reduction, orthopedic methods, topical medications, antibiotics, or therapeutic footwear.

The contractor will comply with all NOBH, State, and Federal policies regarding patient treatment documentation. Additionally, the contractor must complete credentialing and privileging procedures as outlined in the NOBH Medical Staff By-Laws.

The contractor is required to maintain unrestricted licensure in the State of Ohio and participate in Quality Assurance and Process Improvement initiatives. Furthermore, the contractor must adhere to NOBH rules and regulations, performance standards, and the applicable conditions of participation set forth by JCAHO and Medicare (CMS), as well as the medical department's continuous quality improvement plan
Process
MHAS’s evaluation process of responses submitted to this request may consist of up to four distinct phases:
1.  MHAS’s initial review of all proposals for timely submission
2.  An evaluation committee review of the proposals for defects         and scoring
3.  MHAS’s request for more information (clarifications,               interviews, presentations, and/or demonstrations)
4.  Negotiations or best offer requests.

At its sole discretion, MHAS will determine whether phases three and/or four are necessary under this RFP, reserving for itself the ability to eliminate or add phases three or four at any time during the evaluation process. MHAS may add or remove sub-phases to phases 2 through 4 at any time if MHAS believes doing so will improve the evaluation process.
